Job Description

Hourly wage $22.00 - $24.20 ($40,040 - $44,044 a year)

35 Hour work week

Job Duties:

The successful candidate will be expected to:

  • Serve as primary contact for all Fiscal Intermediary related matters associated with caseload.
  • Attend required internal and external job-related trainings.
  • Obtain required documentation from internal/external customers and inform supervisor in a timely manner if requests for documentation are not responded to.
  • Act as liaison between Participant/Designee/Circle of Support (COS) and internal SD departments to identify and resolve any issues with reimbursements and/or staffing associated with Participant budget.
  • Responsible for completing audit of own caseload to ensure quality service delivery and compliance with all SD requirements. Assist each Participant/Designee/COS with implementation of plan/budget to ensure compliance with all applicable SD rules and regulations.
  • Write Staff Action Plans and upload into electronic system if needed.
  • Monitor monthly budget expenditures to ensure Participant does not exceed approved SD budget.
  • Attend mandatory annual and semi-annual Life Plan meetings and complete required notes for file.
  • Support individuals with disabilities to utilize their services and funds to live in the community.
  • Communicate (primarily via phone, email or zoom) with the individuals and their support team, assisting them to utilize their budget according to OPWDD rules and regulations.
  • Obtain and maintain required documentation. Review all documentation for accuracy and guidelines adherence.
  • Effectively perform assigned duties in a remote work environment while maintaining productivity, responsiveness, quality standards, and compliance with Agency policies and procedures.
  • Document all communications.
  • Maintain confidentiality of all work-related information.

Education:

  • High School diploma or equivalent required.
  • College degree in Human Services or related field preferred.

Knowledge, Skills & Abilities:

  • Two years’ customer service in a fast-paced work environment with one year’s experience working in an environment requiring strong attention to detail and the ability to make some decisions independently.
  • At least one year’s experience in a human services organization is preferred. Strong customer service and computer skills.
  • Must be proficient in Microsoft Office suite applications for preparation of documents, spreadsheets and data management using Excel.
  • Sensitivity and awareness of issues related to cultural diversity.
  • Basic knowledge of human services, OPWDD regulations, Medicaid, disability awareness, understanding of the Independent Living philosophy, and case management is preferred.
  • Strong computer skills are required.
  • Strong verbal and written communication, attention to detail, problem solving, excellent organization, time management, and ability to work with a diverse population of individuals is required.
  • Must be able to work independently and initiate work in a fast-paced environment.
  • Excellent Customer Service skills required.
  • Must demonstrate the ability to effectively prioritize workload, manage competing deadlines, work independently, and maintain productivity in a remote work environment.
  • Must be flexible when needed to accommodate meetings outside regular business hours.
  • Maintain a private workspace to protect PHI.
  • Be available to report to the office when required for meetings, training, or other business needs.
  • Maintain a reliable internet connection and notify supervisor of any internet issues immediately.
  • Be accessible to your supervisor for work related communication during working hours.
  • Maintain regular and reliable attendance and actively participate in required meetings, trainings, supervision sessions, and Agency activities.
  • Maintain a professional work environment suitable for conducting confidential participant, employee, and Agency business.
