The Wound Care Registered Nurse (RN) leads the facility's wound care and skin integrity program by providing expert assessment, treatment, prevention, and clinical oversight for residents with acute and chronic wounds. Working collaboratively with physicians, nursing staff, therapy, dietary services, and the interdisciplinary care team, this role promotes evidence-based wound management, regulatory compliance, and exceptional resident outcomes through compassionate, resident-centered care.

• Perform comprehensive skin and wound assessments upon admission, weekly, and whenever clinically indicated.
• Assess, stage, document, and monitor pressure injuries, surgical wounds, diabetic ulcers, venous ulcers, arterial ulcers, skin tears, and other complex wounds.
• Develop, implement, and update individualized wound care treatment plans in collaboration with physicians and the interdisciplinary team.
• Perform wound treatments and dressing changes in accordance with physician orders, evidence-based practice, and facility protocols.
• Monitor wound healing progress and recommend treatment modifications to optimize resident outcomes.
• Lead weekly wound rounds and actively participate in interdisciplinary care conferences.
• Educate residents, families, and team members on skin integrity, pressure injury prevention, wound care, and treatment plans.
• Serve as the facility's clinical resource for wound management and prevention strategies.
• Maintain accurate, timely, and compliant documentation, including wound measurements, assessments, photographs (per facility policy), and treatment records.
• Support infection prevention initiatives and ensure compliance with CMS, state regulations, and facility policies.
• Monitor wound care supplies, assist with inventory management, and participate in Quality Assurance and Performance Improvement (QAPI) initiatives.
• Respond promptly to changes in resident condition and communicate findings to physicians and nursing leadership.

• Current Registered Nurse (RN) license in the State of Kentucky.
• Current CPR/BLS certification.
• Minimum of one (1) year of nursing experience.
• Strong clinical assessment, critical thinking, and documentation skills.
• Knowledge of wound assessment, staging, treatment modalities, and skin integrity management.
• Excellent communication, organizational, and interpersonal skills.
• Ability to work independently while collaborating effectively with an interdisciplinary healthcare team.
• Proficiency with electronic medical records (EMR).
• Ability to meet the physical demands of the position, including lifting up to 50 pounds, prolonged standing and walking, bending, performing wound treatments, and assisting residents with mobility and positioning.

• Experience in skilled nursing, long-term care, rehabilitation, or post-acute care.
• Previous wound care or skin integrity management experience.
• Wound Care Certification (WCC, WOCN, OMS, or equivalent) or willingness to obtain certification within an established timeframe.
• Knowledge of CMS regulations, infection prevention, Quality Measures, and regulatory compliance.
• Experience participating in QAPI initiatives and interdisciplinary care planning.
• Passion for improving resident outcomes through evidence-based wound prevention and treatment.
