Under the supervision of the Director of Nursing, the RN will be responsible for performing nursing functions in the areas of triage, physical assessment, patient education, medical charting, and the provision of ordered injections, consistent with the policies of the center and appropriate regulations of New York State. This position will also assist with the training of LPNs, Medical Assistants, and the HIV Clinical Coordinator in appropriate clinical and chart review activities.
Essential Job Functions
The following duties are mandatory requirements of the job:
- Provides direct patient care within scope of practice to ensure implementation of plan of care.
- Reports on all necessary and/or unusual occurrences in accordance with established policies and procedures.
- Administer prescribed medications as per provider’s orders and ensures proper documentation in the patient’s chart.
- Reports unusual findings to providers and ensures proper follow up.
- Assist providers in advanced medical procedures and treatments when necessary.
- Ensures proper DON reporting as per clinic policies and procedures.
- Provides patient teaching when necessary.
- Ensure that sterilization of equipment and supplies using germicides, sterilizers, or autoclaves are done.
- Participates in the development of Quality Improvement/Assurance Programs.
- Assist in the upkeep of all nursing logs as needed.
- Monitors patient arrival and assist with patient flow, monitors cycle times.
- Obtain and record patient’s information, data and record histories, vital signs, venipuncture, allergies, specimen collection, and documents results in the patient’s EHR.
- Perform, prep, and assist as needed with clinical and technical services such as EKG, Phlebotomy, tests, finger sticks, BMI, hearing and Vision tests, wound care, blood glucose, administer medications and treatments.
- Practice accepted infection control measures in accordance with OSHA Blood Borne Pathogens Standards and Center Policies and Procedures.
- Attends and participates in Center’s meetings and training as required and assigned.
- Participate in staff development and educational activities.
- Supervises and trains LPNs and Medical Assistants
- Assist with administrative tasks, which may include prior authorizations, form completion, referrals, maintaining logs and educational materials and sending/receiving messages, mail, faxes or records, labs, x-ray reports and hospital notes.
- May discuss lab results with patients.
- Collaborate with clinicians to develop individualized patient care plans and ensure that patients follow care plans.
- Care Coordination- Supervises referral tracking, hospitalization tracking, lab tracking processes.
- Other duties as assigned by the Director of Nursing
Qualifications
Education and Certification
- Possession of a current and valid New York State License as a Registered Nurse, and evidence of appropriate education equivalent to the Associate Arts Degree in Nursing or bachelor’s degree in nursing
- Previous experience in a licensed Ambulatory health care facility and a minimum of one year of direct RN experience preferably in primary care, Community Health Center, or Clinic. Experience and knowledge of HIV/AIDS is essential.
- BLS Certified
- Demonstrated ability in performance as a Registered Nurse as evidence through work reference from previous employers.
- Culturally Competent with skills/experience working with diverse populations.
- Ability and desire to provide services to all life cycles consistent with New York State Health Department and the Bureau of Primary Health Care Program (Federal) guidelines.
Special Skills and Knowledge
In addition to the above-listed job responsibilities and educational requirements, the ideal candidate for this position possesses most or all the following:
- Must be available to work weekends and late nights
- Maintain HIPPA- patient/employee confidentiality
- Knowledge of EMR system a must
- Responsible for participation and cooperation with Corporate Compliance Matters
- Excellent interpersonal skills must demonstrate professionalism, courteous and respectful attitude in dealing with patients, families, significant others, and team members.
- Display courtesy, tact, and patience with all members of the team and extended community
- Ensure that all patients and visitors receive personalized prompt attention and are treated with receptiveness, dignity, and respect
- Contacts- ongoing interaction with all providers, patients, Medical Director, CEO, QI committee and support staff
- Provide instructions to the patient upon discharge as instructed by providers
- Strong organizational skills and ability to think clearly and to work in a busy clinic
- Strong skills in Microsoft Outlook and Word
- Excellent verbal and written communication, and time management skills
