In Home Services Coordinator for Family Caregiver, OPI, and OPIM at South Coast Business Employment Corporation – Coos Bay, Oregon

This position is responsible for the care coordination of Community Base Service programs, Responsible for the successful implementation and maintenance of the Older American’s Act programs in Coos County. Coordinates outreach services and coordinates information for OAA services.

Fulfill agency mission by provisioning services available through the Area Agency on Aging including in-home services, the Employer Resource Connection program, Oregon Project Independence (OPI), and others, thereby helping to ensure seniors and people with disabilities thrive in Coos county. This job is performed at main office and in the field requiring travel to clients in a variety of settings, homes, facilities, etc.􀀁 The service area is Coos county.
