Nurse Practitioner Palliative Care - Hybrid at Our Hospice Of South Central Indiana – Columbus, Indiana

Schedule: Clinic/Hybrid Palliative Model (4 days per week / 10 hour days)

Professional Setting: Out-patient Palliative Care Clinic

The Palliative Care Nurse Practitioner (NP) provides advanced practice nursing care to patients with serious, chronic, or life‑limiting illness in an outpatient clinic setting. You will serve as a consultant to referring physicians to assist managing patient’s symptoms and providing goals of care conversations with patients and families. You will also serve as a resource and advisor to the clinical staff and leadership to ensure that our patients receive the highest quality of care.
