JOB SUMMARY: The Registered Dietician is responsible for the education of patients, their families and appropriate support systems about diabetes self-management and related issues as well as other chronic conditions. The Registered Dietician provides diabetes self-management education (DSME), Medical Nutrition Therapy (MNT), and coordinates protocol-based diagnostic testing and treatment, which may include medication management, nutrition, physical activity, healthy coping, risk reduction, monitoring and problem-solving. The Registered Dietician is responsible to ensure the program is meeting the criteria to gain national accreditation. The RD CDCES also serves as a resource to other staff members as needed.
Requirements:ESSENTIAL FUNCTIONS:
- Reviews clinicians' diagnosis and designs and implements therapeutic nutritional care plans for patients utilizing general dietetic standards and regulations.
- Provides direct patient education, curriculum developed for diabetes education.
- Teaches technical stills, concepts of self-management and facilitates behavior change for patients with Type, Type 2, and gestational diabetes as well as those with pre-diabetes.
- Individualizes education presentation materials to the patient and patient circumstance.
- Speaks to patients to learn about their goals and dietary needs, preferences, and restrictions.
- Assesses patient's cultural, psychosocial, behavioral, and educational needs (to self-management, nutrition, physical activity, medication taking, health maintenance labs) and interprets the information to individualized clinical and educational needs.
- Develops nutrition plans for patients diabetes, renal disease and other chronic conditions.
- Collaborates with the patient to individual goals.
- Compiles information and tracking patient towards their health goals.
- Communicates with providers and nursing staff keep them aware of patient progress and recommends changes in regimen as appropriate.
- Evaluates the patient's comprehension of education and develops follow up plans to address ongoing needs.
- Documentation of required interventions, evaluations, and follow-up is consistent with departmental and ADA standards.
- Maintain CDESC accreditation.
KNOWLEDGE, SKILLS AND ABILITIES:
- Basic PC skills, Epic electronic medical system
- Experience with blood glucose monitors continuous glucose monitors
- Knowledge of diabetes pharmaceuticals
- Excellent written and verbal communication skills
- Knowledge of and ability to partner with patient populations
- Excellent time management skills and strategies
EDUCATION/EXPERIENCE:
- Bachelors’ Degree in Dietetics or similar required
- At least 5 years of experience as a Dietician is preferred
- At least 3 years of experience as a diabetes care and education specialist is preferred
- Experience with continuous glucose monitoring injectable diabetes medications
CERTIFICATES, LICENSES, REGISTRATIONS:
- Registered Dietician (RD-Commission of Dietetic Registration) and Dietician (LD-MN Board of Dietetics and Nutrition Practice) is required
- Current Certified Diabetes Educator (CDE) certification is preferred
PHYSICAL DEMANDS:
- Prolonged periods of sitting at a desk and working on a computer
- Must be able to lift up to 15 pounds at times
WORK ENVIRONMENT:
Work is performed in a clinic office environment. Contact with staff, patients and outside agencies. Possible exposure to communicable disease and medical preparations common to clinic environment.
SUPERVISORY RESPONSIBILITIES:
- None
Hours: Part -Time 0.40 FTE, 16 hours/week. Not eligible for benefits.
