About This Position

Case Manager II Family Preservation (Diversion Truancy Program)

Diakon is one of the premier employers in Pennsylvania and Maryland. We are a non-profit organization which includes Diakon Child, Family, and Community Ministries.
General Responsibilities:
  • Responsible for serving as a resource person, case manager, counselor, and trainer to support the youth and families of the Diversion Truancy program.
  • Maintains a caseload of five (5) youth and families.
  • Establishes and maintains professional and supportive relationships with youth, birth families, referring personnel, treatment providers, school districts and others involved in the lives of children and youth.
  • Schedules, completes paperwork necessary (ISP), and participates in initial Treatment Team meetings and quarterly reviews.
Education/Training: A BA or BS degree in social service or a related field of study normally required
Experience: A minimum of one (2) year’s experience in a related child welfare or human service position normally required.
ADA: ability to bend, stoop, sit, stand, reach and lift items weighing up to 50 lbs.; valid driver's license; visual and auditory skills.

In addition, Diakon offers a Love of Thy Neighbor Fund, which provides a per-day living expense and matches up to five PTO days for approved service or mission trips with other nonprofit organizations.
