POSITION DESCRIPTION:

Responsible for providing medical care to patients presenting themselves at any of SIHF Healthcare’s facilities, as reasonably scheduled. This shall include any existing or future medical facility site as mutually agreed upon, provided that any new site is within reasonable commuting distance of the existing sites.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

1. Maintain continuity of care, which may include providing direct care, in the event a patient is transferred to a long-term care facility.

2. Adhere to and follow state and federal guidelines implemented for patient care at long term care facilities. If unable to adhere to said guidelines, the patient must be referred to the appropriate provider on staff at the facility.

3. Adhere to the scheduled provider appointment hours.

4. Satisfy the relevant and applicable requirements for the State of Illinois Medicaid managed care plan as needed.

5. Hospitalize and treat routine medical patients, and maintain appropriate staff privileges.

6. Complete in a timely manner, all outpatient records and all required inpatient records for said Hospital(s).

7. Participate in staff peer review of a clinical practice in order to assess the quality of care and utilization of resources.

8. Perform such reasonable administrative and managerial duties, including, but not limited to, health center management and quality assurance activities.

9. Provide any other services as may reasonably be requested.

KNOWLEDGE, SKILLS, ABILITIES:

1. Ability to accommodate, along with other group physicians, “on-call” rotations.

2. Ability to meet with the Medical Director and Administration, as needed, to review expected office encounter productivity goals.

3. Ability to maintain harmonious working relationships with members of the Medical Staff, other employees, and the patients served.

4. Must possess strong interpersonal skills to deal tactfully and courteously with patients, providers, co-workers, subordinates, and others.

5. Must be professional in appearance and demeanor.

EDUCATION AND EXPERIENCE:

MD or DO in chosen area of specialty.

COMPLIANCE STATEMENT:

Abide by the requirements of all applicable State and Federal laws.

MEDICAL HOME STATEMENT:

Be aware and understand expectations and responsibilities of the Patient Centered Medical Home. Understand role, complete relevant duties, and the requirement to maintain highest level of proficiency within scope, as part of this team-based approach in the patient care continuum.

PHYSICAL DEMANDS:

Light physical activity performing non-strenuous daily activities of an administrative nature.

WORK SCHEDULE:

Normal work hours for this position are Monday through Friday dayshift unless otherwise specified. Some Saturdays and evenings may be included as necessary to meet the needs of treated patients and to ensure continuity of care.
