Description:

The Youth and Young Adult Transitional Housing Case Manager provides intensive, strengths-based case management for young people, ages 18–24 experiencing homelessness and residing in One80 Place transitional housing program. This role helps youth achieve stability and self-sufficiency by coordinating housing supports, life-skills development, education and employment pathways, and access to community resources. The Case Manager builds trusting, trauma-informed relationships that empower youth to set and achieve housing stability goals. This position may work with young adults who have exited the Department of Juvenile Justice, the foster care system or pregnant or parenting youth, and all other populations of youth experiencing homelessness. The Transitional Housing Case Manager will work in a combination of settings – in office, community based, and in One80 Place’s Transitional Housing and emergency shelter locations. This position may require evening or occasional weekend hours based on youth needs.

This position is funded in whole by One80 Place’s Youth Homelessness Demonstration Program grant.

One80 Place ends and prevents homelessness throughout South Carolina with offices located in Charleston and Columbia. This position will be based in Charleston.

STATUS: Full-time, Regular / Exempt

Requirements:

SPECIFIC REQUIREMENTS:

1. Bachelor’s degree in social work, human services, psychology, counseling, or related field preferred OR equivalent experience.

2. Experience working with youth or young adults who have experienced homelessness, trauma, foster care involvement, or system involvement.

3. Demonstrated ability to build rapport with individuals experiencing homelessness.

4. Strong crisis intervention, communication, and conflict-resolution skills.

5. Ability to maintain boundaries while offering compassionate, strength-based support.

6. Valid driver’s license and reliable transportation.

7. Knowledge of trauma-informed care, positive youth development, harm reduction, motivational interviewing, or housing-first principles.

8. Experience in transitional housing, rapid rehousing, or supportive housing programs.

9. Familiarity with community resources, social service systems, and coordinated entry processes.

DUTIES AND RESPONSIBILITIES:

Case Management & Support

1. Conduct comprehensive intake assessments, including housing history, safety needs, strengths, risks, and service goals.

2. Develop individualized service plans with youth that include short- and long-term goals.

3. Meet regularly with residents to provide coaching, crisis intervention, and problem-solving support.

4. Maintain a trauma-informed, youth-centered, and culturally responsive approach in all interactions.

5. Support youth in building life skills such as budgeting, conflict resolution, time management, healthy relationships, and self-advocacy.

Housing Stability & Program Coordination

1. Support youth in transitioning into safe, stable housing and maintaining compliance with program requirements.

2. Facilitate house meetings, workshops, and group activities that build community and promote skill development.

Education, Employment & Resource Connection

1. Assist youth in accessing education pathways, job training, and career development opportunities.

2. Connect residents to mental health services, medical care, benefits, childcare, and other essential supports.

3. Coordinate referrals and maintain strong partnerships with schools, workforce programs, and community providers.

Documentation & Compliance

1. Maintain accurate and timely case notes, assessments, and progress reports HMIS.

2. Ensure compliance with all grant requirements and program policies.

3. Participate in audits, quality assurance reviews, and reporting as needed.

Collaboration & Teamwork

1. Work closely with program staff, supervisors, outreach teams, and community partners.

2. Participate in case conferencing, staff meetings, and team meetings.

3. Contribute to a safe, positive, and supportive environment within the housing program.

4. Attend relevant training on topics impacting homeless youth.

PHYSICAL, ENVIRONMENTAL AND SENSORY DEMANDS:

  1. Corrective vision and hearing to normal range.
  2. Ability to move between service locations
  3. Ability to lift 50 lbs.
  4. Ability to relate effectively to individuals experiencing homelessness.
  5. Possible exposure to communicable diseases and vicarious trauma.
  6. Requires sound mental reasoning, sound judgment, and the ability to respond calmy and effectively in a crisis.
