Southland Healthcare and Rehab is currently seeking compassionate, dependable, and motivated Licensed Practical Nurses (LPNs) and Registered Nurses (RNs) to join our nursing team for full-time 12-hour shifts. Essential Job Duties and Responsibilities

Provide direct nursing care to residents in accordance with care plans, physician orders, and facility policies

Administer medications and treatments accurately and timely

Monitor residents for changes in condition and report concerns appropriately

Assess residents and communicate relevant findings to physicians, families, and the interdisciplinary team

Supervise and support Certified Nursing Assistants and other assigned staff

Ensure proper documentation of care, treatments, incidents, and resident status

Respond promptly and professionally to resident needs, emergencies, and call lights as needed

Review, implement, and update resident care plans as appropriate

Maintain infection control and safety standards throughout the facility

Participate in admissions, discharges, transfers, and other resident care coordination activities

Provide resident and family education when needed

Work collaboratively with team members to promote positive clinical outcomes and resident satisfaction

Ensure nursing services are delivered with dignity, compassion, and respect

Requirements

Current and active LPN or RN license in the State of [Insert State]

Current CPR certification preferred

Previous experience in long-term care, skilled nursing, rehabilitation, or post-acute care preferred, but not required

Strong clinical assessment and critical thinking skills

Ability to supervise staff and work effectively as part of a team

Excellent communication and documentation skills

Dependable attendance and professional conduct

Ability to work 12-hour shifts and meet the physical demands of the position

Must pass required background screening and any additional pre-employment requirements in accordance with facility policy and state regulations

Preferred Qualifications

Experience with medication administration and treatment documentation

Knowledge of long-term care regulations and standards

Strong organizational and time-management skills

Compassionate, resident-centered approach to care

Ability to remain calm and effective in a fast-paced environment
