Job Description:
Chesapeake Integrated Behavioral Healthcare (CIBH) Assertive Community Treatment (ACT) team is looking for a Field Case Manager who’s ready to step into the field, think on their feet, and make a real impact with individuals living with serious mental illness. This Field Case Manager role isn’t traditional outpatient therapy.

As a Field Case Manager, you’ll meet people where they are—literally. Homes, shelters, streets, and community settings become your “office” as you help individuals stabilize, recover, and reclaim independence.
Duties Include but are not limited to:
- Develops and implements an individual program for service recipients including referral and coordination of care/services or placement, and/or direct therapy; ensures access to resources, services, and support for family.
- Provides community-based case management, vocational, and housing support, crisis stabilization/intervention, and skill-building activities.
- Serves as liaison between service recipients and other care providers such as physicians and therapists.
- Provides service techniques including rapport-building strategies, facilitating meeting basic needs, and motivational interviewing techniques.
- Documents all services in the electronic medical record system.
- Maintains records for area of responsibility in accordance with departmental, state, and federal regulations, laws, policies, and procedures; processes daily paperwork including progress notes, insurance forms, daily time sheets, service logs, and billing reports.
- Develops individualized treatment plans that respond to the needs of the person served and coordinates service delivery that coincides with the treatment plan.
- Enhances professional development to incorporate evidence-based best practices in rendering individual care.
- Attends scheduled staff meetings and ACT daily team meetings to review and plan services.
- Performs other related duties as assigned.
- Serves as ALPHA personnel to staff city shelters when opened/needed.

Required Qualifications:
Vocational/Educational Requirement: Requires a bachelor’s degree in social work, counseling, psychology, or a closely related field or other degree as approved by the Virginia Department of Health Professions. A master’s degree in Social Work, Counseling, or Rehabilitation is required for some assigned programs.
Experience: In addition to satisfying the vocational/educational standard, this class requires a minimum of two years of related, full-time equivalent experience.
Special Certifications and Licenses: Requires a valid driver’s license and a driving record that is in compliance with City Driving Standards.
Requires a Qualified Mental Health Professional (QMHP) or Qualified Intellectual Disability Professional (QIDP) certification and highest Part C standards within 6 months of hire; CPR, First Aid and medication administration certifications within 3 months of hire, Certified Substance Abuse Counselor (CSAC) certification upon hire, Case Management Modules and Intellectual Disabilities Workbook completed within thirty days of hire, Part C Early Intervention Certification within six months of hire, and/or Pre-Screening Certification within 6 weeks of hire (12 weeks for part-time staff).
Special Requirements: Employees may be expected to work hours in excess of their normally scheduled hours in response to short-term department needs and/or City-wide emergencies. Emergency operations support work and work locations may be outside of normal job duties.

Preferred Qualifications:
- Master’s degree in psychology, social work, counseling or a closely related field, or other Human services degree as approved by the Virginia Department of Health Professions.
- Experience in Assertive Community Treatment (ACT)
- Experience with supported employment and vocational training
- Experience with providing case management services
