Therapist Residential Clinical and Aftercare at ST ANNE INSTITUTE – Albany, New York

Salary range:

$48K-58K

40 hours per week work schedule: Sunday through Thursday OR Tuesday through Saturday including some evenings.

Job Description:

Under the direction of the Director of Residential, Clinical and Aftercare Services, the therapist will provide individual, family, group therapy and aftercare services to assigned residential and aftercare clients and their families, while corresponding and collaborating with outside agencies to support the clients’ goals and to maintain community and family connections.

Responsibilities:

Develop comprehensive clinical assessments on assigned individuals and families

Serve as the ongoing liaison with community agencies and referral sources

Complete discharge summary and assist with discharge planning on each assigned case

Provide aftercare services for assigned cases post-discharge

Meet at least once a week with treatment team members in rounds and/or unit meetings to coordinate treatment planning.

Requirements and Education:

Masters level social worker or mental health counselor, LMSW, LCSW, MCH, LMHC; experience working with individuals, families and groups preferred; recent Masters level graduates encouraged to apply; ability to function as part of a multidisciplinary team; valid driver’s license required.
