As a Licensed Field Nurse, you will provide comprehensive, patient-centered care tailored to each client’s individualized service plan. You will play a critical role in promoting independence, ensuring safety, and enhancing quality of life for our clients and their families.
Key Responsibilities Direct Patient Care
- Provide compassionate personal care including bathing (bed bath, shower, or tub), dressing, grooming, oral care, and hygiene support
- Assist with mobility including ambulation, wheelchair use, and safe transfers
- Support toileting needs, including catheter and ostomy care
- Reposition bedridden clients and maintain clean, comfortable bedding
- Assist with meal preparation and support nutritional intake as needed
Clinical Responsibilities
- Monitor and record vital signs (heart rate, blood pressure, respiratory rate, temperature)
- Document intake and output
- Administer and manage medications as prescribed
- Assess client nursing needs on an ongoing basis
- Provide nursing services in accordance with the established care plan
- Promptly report changes in condition, concerns, or progress to the Case Manager and physician
- Complete accurate and timely clinical documentation
Care Plan Collaboration
- Participate in the development and implementation of individualized service plans
- Observe and report changes in mood, behavior, or emotional status
- Communicate effectively with interdisciplinary team members
Supportive Services
- Perform light housekeeping to maintain a safe, healthy environment (laundry, vacuuming, dusting, kitchen tasks)
- Organize client living spaces to promote safety and comfort
- Assist with errands and transportation, including medication pickup and grocery shopping
Professional Standards
- Practice strictly within legal scope of licensure
- Maintain professional appearance and boundaries
- Promote teamwork, unity, and a culture of respect
- Adapt to additional responsibilities as assigned by leadership
Working Environment
- Daily travel to client homes and occasional visits to the agency office
- Flexible scheduling, including potential weekends and holidays (with compensation provided)
- Must be able to lift at least 25 pounds
- Work environment varies based on individual client needs
Minimum Qualifications
- Current, valid RN or LPN/LVN license
- Reliable personal vehicle with valid insurance
- Clear criminal background check and negative pre-employment drug screening
- Strong verbal and written English communication skills
