Position Overview We are seeking a licensed Physical Therapist for full-time and PRN home health visits. This role is ideal for clinicians who value autonomy, meaningful patient relationships, and manageable caseloads.

Key Responsibilities Perform in-home physical therapy evaluations

Develop and implement individualized treatment plans

Provide one-on-one therapy to improve mobility, strength, and independence

Educate patients and caregivers on exercises and safety

Monitor progress and adjust care plans as needed

Complete timely and accurate EMR documentation

Collaborate with interdisciplinary care teams

Supervise PTAs as required

Ensure compliance with state and federal regulations

Qualifications Active Virginia Physical Therapist license

Graduate of an accredited PT program

Minimum 2 years of experience (home health or SNF preferred)

Strong clinical and communication skills

EMR experience preferred (WellSky or HCHB)

OASIS experience a plus

Valid driver’s license & reliable transportation
