Responsibilities

Conduct comprehensive nursing assessments in patients’ homes

Develop and update individualized care plans

Provide skilled nursing care including wound care, medication management, infusion therapy, catheter care, and palliative care

Monitor patient progress and report changes to physicians and care teams

Educate patients and caregivers on disease management and self-care

Document care accurately in the EHR system

Collaborate with interdisciplinary team members

Follow infection control, safety, and emergency protocols

Qualifications

Active RN license in the state of employment

Minimum 2 years of nursing experience (home health preferred)

Strong assessment, clinical, and communication skills

Experience with wound care and skilled nursing procedures preferred

Proficient with electronic health records

Valid driver’s license and reliable transportation
