About This Position

South Central Mental Health (SCMH) is seeking full-time Children’s Case Managers to serve our El Dorado and Andover/Augusta/Wichita catchment areas. We are committed to delivering high-quality care and recognize that this begins with hiring dedicated, skilled professionals.

The Children’s Case Manager provides services within our Community-Based Program, supporting children diagnosed with serious emotional disturbances and helping them and their families access the resources needed for stability and success.

South Central Mental Health is currently recruiting Children's Case Managers. Benefits include 401(k) participation, 401(k) 5% salary match after one year of employment, eligible for paid vacation after six months of employment, paid sick leave accrual begins at hire date, employer paid life insurance and LTD after one year of employment, low-cost employee health insurance options, no cost employee dental and vision coverage, and 10 paid holidays per calendar year.

GENERAL RESPONSIBILITIES:

Provide one-on-one service to consumers who have a severe emotional disturbance. Overall endeavor is to prevent hospitalization or out-of-home placement and to reintegrate consumers into the community. Specialist is an active participant in meeting related administration or treatment planning processes.

Judgment: Sound judgment is used in ongoing assignments. Maintains a positive, professional and supportive relationship with consumers. Maintains a positive and professional relationship with staff from SCMH and other agencies. Confidentiality is maintained at all times. Specialists keep other staff informed of unusual situations regarding consumers.

Documentation: Progress notes are accurate and reflect progress toward treatment goals. Documentation is current and turned in weekly.

Essential Job Functions:

  1. Develop continuous supportive relationships with identified clients and their families.
  2. Assist with targeted case management/care coordination duties as needed.
  3. Be in contact with designated clients and families for regularly scheduled appointments to meet the client and families needs as dictated on their plan of care. Hours vary by client need.
  4. Maintain direct client and family contacts in their natural environment based on needs.
  5. Help clients learn more effective controls to manage their mental health diagnosis.
  6. Follow the instructions of the clinical supervisor regarding effective intervention strategies.
  7. Report to team leader and client’s therapist all concerns regarding clients change in behavior or reportable issues.
  8. Update client’s goals, objectives, and services in the treatment plan if needed and follow-up with the therapist for their clinical oversight.
  9. Facilitate client autonomy by helping them build on strengths, increase community integration and learn more effective controls for emotional disturbances based on medical necessity.
  10. Build a relationship with community agencies that will provide a natural, supportive network for clients in their quest for mastery in various situations; liaison
  11. Assume an advocacy role for clients and their families.
  12. When necessary or possible, help integrate clients into existing emergency service systems.
  13. Attend and complete training sessions as required by SCMH.
  14. Have a minimum of 57.5% of work hours of direct contact with clients per week.
  15. Perform other duties as directed by supervisor.

Minimum Qualifications:

Have at least a BA/BS degree or be equivalently qualified by work experience or a combination of work experience in the human services field and education, with one year of experience substitution for one year of education. Must have the ability to communicate positively and effectively with others, and to work independently as well as be a productive part of a team. Must have a dependable vehicle and valid driver’s license. Must provide proof of insurance. Pass a criminal background and driver’s license check to the satisfaction of SCMH.
