Home Health Physical Therapist at One Home Health LLC – Chicago, Illinois

We are seeking a Home Health Physical Therapist to join our team in Downtown, Lakeview, Edgebrook, Streeterville, Norridge, Harwood Heights, Humboldt Park! You will plan physical rehabilitative programs to improve patient quality of life.

Responsibilities:

• Develop and implement physical therapy programs that focus on rehabilitation
• Work directly with patients to achieve maximum physical recovery
• Evaluate effects of therapy treatment and communicate patients' progress
• Record and document patient care services
• Collaborate with other team personnel to achieve well-rounded care
• Work professionally with all levels of staff members
• Ensure compliance in all aspects of patient care and medical documentation
• See patients as directed by Vice President of Clinical Operations

Qualifications:

• Previous experience in home health care
• Ability to communicate effectively
• Ability to handle physical workload
• Ability to build rapport with patients
• Excellent written and verbal communication skills
• Strong leadership qualities
• Doctorate in Physical Therapy
