About This Position

Reports to the Manager of Case Management. Works collaboratively to continually improve the processes of assessment, planning, facilitation, care coordination, evaluation, and advocacy for options and services to meet patients’ and families’ comprehensive health needs. Assists with the development of a standardized care coordination model throughout Beacon to improve patient care.

Case Management Practice
  • Develops processes to assess the effective and efficient delivery of case management and discharge planning services to patients and families.

  • Validates that services provided to patients are consistent and individualized.

  • Evaluates the effectiveness of case management associates and services.

  • Establishes and supports quality improvement programs (unit-based and hospital-wide).

  • Acts as a liaison between community agencies, case management staff, and the organization.

  • Serves as an expert resource to staff and supports effective problem-solving in complex discharge planning or special situations, such as protective services referrals, Level II reviews, adoptions, etc.

  • Role models excellence in the provision of case management, discharge planning, and social work services, and supports departmental staffing needs when indicated.

Management
  • Manages human, fiscal, and other resources to achieve departmental and organizational goals.

  • Ensures effective and appropriate utilization of all available resources.

  • Ensures appropriate staffing to support unit acuity levels and operational functions.

  • Responsible for effective coaching, counseling, evaluation, and education of staff to ensure excellent performance.

  • Monitors associate documentation and provides feedback.

  • Develops unit-specific goals reflective of hospital-wide mission and goals.

  • Demonstrates compliance within areas of responsibility for professional, regulatory, and governmental standards of care.

  • Maintains knowledge of and supports organizational policies and procedures.

  • Investigates, documents, and promptly reports any risk management, quality, or utilization issues to administration.

  • Participates in self-development through Beacon to enhance management skills.

Human Resource Development
  • Promotes an environment that maximizes associate engagement.

  • Investigates and documents all associate issues thoroughly.

  • Develops programs and provides support for continued professional development of staff.

  • Ensures associates have achieved competency to carry out designated case management, discharge planning, and social work roles and responsibilities.

  • Facilitates collaborative relationships among disciplines and departments to ensure effective, high-quality service delivery.

  • Acts as a professional role model in communication, problem-solving, team building, and conflict resolution.

Contribution to Department Effectiveness
  • Completes other job-related duties and projects as assigned.

Organizational Responsibilities

The associate complies with the following organizational requirements:

  • Attends and participates in department meetings and is accountable for all information shared.

  • Completes mandatory education, annual competencies, and department-specific education within established timeframes.

  • Completes annual employee health requirements within established timeframes.

  • Maintains license, certification, and registration in good standing throughout the fiscal year.

  • Direct patient care providers maintain current BCLS (CPR) and other certifications as required by position or department.

  • Consistently utilizes appropriate universal precautions, personal protective equipment, and ergonomic techniques to protect patients and self.

  • Adheres to regulatory agency requirements, survey processes, and compliance standards.

  • Complies with established organizational and departmental policies.

  • Is available to work overtime and additional or alternative shifts and schedules as required.

Education and Experience

The knowledge, skills, and abilities described below are typically acquired through the successful completion of a Bachelor of Science in Nursing or a Bachelor’s degree in Social Work. Must be a licensed Registered Nurse or Social Worker in the State of Indiana. A minimum of two to five years of job-related experience is required. Management or supervisory experience in case management, utilization management, hospital services, or a related field is required.
