JOB TITLE: Case Manager
PROGRAM AREA: Crisis to Housing Services (Rapid Re-Housing Program)
JOB OBJECTIVE: Actively provide effective service delivery and supports that help young adults, ages 18-24, quickly transition from experiencing homelessness to stable, permanent housing and attain self-sufficiency. The case manager will work to assess an individual’s needs, providing a housing-first, trauma-informed approach to youth in the Rapid Re-Housing Program.
DIRECT SERVICE
Essential Functions
- Manage a case load of approximately 15-20 young adults including the direct provision and coordination of housing navigation assistance and supportive services.
- Deliver services using a strong trauma informed and positive youth development approach.
- Assist in preparing young adults for housing, including obtaining any necessary documentation and providing life skills training.
- Assist young adults in creating housing plans and provide support in goal attainment, having a strong focus on financial management/budgeting and tenant rights and responsibilities.
- Work cooperatively with the Manager of Housing Partnership to communicate housing needs and assist in successfully locating appropriate housing.
- Learn and utilize HMIS and Salesforce data management system to document interaction with young adults in a timely, consistent, and ethical manner.
- Transport young adults as needed for appointments such as identification, health care, housing, education, and vocation.
- Provide information and referral services to young adults about community resources as needed and follow-through with appropriate adjunct services.
- Facilitate problem-solving sessions to overcome obstacles and resolve conflicts involving young adults.
- Assist young adults formally and informally as they master life skills and build self-sufficiency, included but not limited to: job readiness, financial management and budgeting, communication skills, transportation, sexual health, and being a good tenant.
- Help to build and implement strong internal collaborations that maximize Oasis Center’s resources for young people.
- Related tasks as assigned.
ADMINISTRATION
Essential Functions
- Document young adult interactions in a timely, consistent and ethical manner and effectively maintain all client records in Oasis Center’s agency database and in HMIS.
- Attend community meetings for networking and resource updates.
- Participate in staff and program development by attending Oasis Center and community-based trainings
- Support Oasis staff in learning about and accessing resources available in community to connect youth to housing.
- Attend weekly staff meetings and regular supervision, collaborating with supervisor and appropriate staff on specific case-related problems.

- Minimum one year of relevant experience.
- Knowledge and experience with the Housing First and Rapid Re-Housing model, preferred.
- Ability to form strong, caring and affirming relationships with all young people, including those identified as LGBTQ+.
- Strong communicator and problem solver, with the ability to work effectively with young adults, staff, community partners, and other stakeholders.
- Understanding of racial and gender inequities and other forms of discrimination and willingness to operate from an anti-oppression model.
- Knowledge of and ability to apply Positive Youth Development, Trauma-Informed Care, Motivational Interviewing, and other evidence-informed approaches rooted in adolescent and young adult development.
- Knowledge of community resources and the ability to network and collaborate with external partners.
- Valid driver’s license and good driving record.
PHYSICAL/ENVIRONMENTAL REQUIREMENTS
- Ability to effectively communicate in oral and written form.
- Ability to monitor client interaction visually and auditorily.
- Ability to maneuver stairs and uneven terrain.
