About This Position
Reports to Director of Supported Housing and Psychosocial Rehabilitation Services FLSA / WAGE Exempt, $59,963.75 - $64,760.85
JOB SUMMARY
Responsible for oversight of PSR program operations ensuring services are delivered in a trauma-informed, recovery-oriented, and person-centered manner. Ensures compliance with CARF, TennCare, and organizational standards, while promoting member independence, dignity, and quality outcomes.
ESSENTIAL FUNCTIONS
1. Program Operations & Service Delivery.
- A. Ensures members are welcomed and provided resources to support independence and recovery.
- B. Assesses member needs and aligns programming with individualized goals.
- C. Coordinates with internal and external providers for continuity of care.
- D. Ensures groups and services meet clinical and psychosocial rehabilitation standards.
- E. Coordinates across programs to enhance service integration and outcomes.
2. Staff Supervision & Leadership
- A. Provides supervision at least twice monthly; documents all supervision sessions.
- B. Models trauma-informed, recovery-oriented, and person-centered care.
- C. Ensures staff maintain respectful, strength-based communication.
- D. Provides coaching, corrective action, and performance management aligned with HR policy.
- E. Ensures staffing coverage, scheduling, and operational continuity
- D. Leads a weekly Team meeting with direct reports
3. Documentation & Compliance (CARF/TennCare/Licensure)
- A. Ensures all documentation meets CARF, TennCare, and licensing standards.
- B. Conducts weekly documentation audits for completion, accuracy, and billing readiness.
- C. Ensures timely completion of progress notes, service documentation, and incident reports.
- D. Implements corrective action plans (CAPs) when deficiencies are identified.
4. Member Rights, Ethics, and Safety
- A. Ensures all members are treated with dignity, respect, and compassion per Code of Ethics.
- B. Ensures basic needs (food, safety, access to care) are consistently met.
- C. Applies program rules in a manner that supports clinical judgment and member well-being.
- D. Ensures compliance with confidentiality and HIPAA standards.
- E. Responds to incidents and ensures reporting within required timeframes
5. Communication & Coordination
- A. Maintains clear, timely communication with Director and interdisciplinary teams.
- B. Ensures staff receive consistent updates and expectations.
- C. Participates in required meetings and quality improvement initiatives
6. Quality Improvement & Outcomes
- A. Monitors program outcomes and member progress.
- B. Participates in quality improvement initiatives aligned with CARF standards.
- C. Uses data to inform program improvements and service delivery.
7. Cultural Competence
- a. Collaborate with staff, members, and the community to gain the perspectives of others having diverse opinions, abilities, values, beliefs, perspectives, and ethnic or cultural backgrounds.
- b. Take deliberate steps to increase own cultural competency by attending trainings, events, discussions, workshops, etc.
- c. Promotes culturally responsive services and continuous competency development
SKILLS AND QUALIFICATIONS
- 1. Bachelor's Degree from an accredited college in social work, sociology, or other related field.
- 2. One year's experience in a mental health or social service setting.
- 3. Three (3) years of supervisory experience
- 4. Eager to learn.

WORKING CONDITIONS
Works in an office type setting. May go into the community for outings with clients.
PHYSICAL DEMANDS
Must be able to sit and stand intermittently. Must be able to communicate effectively.

SCHEDULE
Monday through Friday either 7:30 am to 3:30 pm, 8:00 am to 4:00 pm, or 8:30 am to 4:30 pm.
WORK LOCATION
Primary work location is 948 Woodland Street, Nashville.
