Isaiah House is seeking a compassionate, organized Discharge Coordinator to help clients transition from inpatient treatment to continued care and recovery support. In this role, you will coordinate aftercare placements and referrals, connect with families, and collaborate with our Clinical, Programs, and Operations teams to support each client’s next steps.

- Arrange aftercare placements and referrals to support clients transitioning from inpatient treatment.
- Monitor program completion dates and collaborate with the appropriate departments to ensure timely discharge planning.
- Develop and maintain strong relationships with referral sources.
- Connect with clients’ families and loved ones to address questions, concerns, and support needs.
- Facilitate clear, timely communication between families and Isaiah House staff.
- Organize monthly onsite and offsite visits.
- Maintain a clean, organized workspace that supports efficient daily operations.
- Follow guidance from Programs leadership and executive staff, and perform additional duties assigned by the immediate supervisor.
Qualifications and Skills
- High school diploma required.
- Strong planning, organizational, and time management skills.
- Effective, professional verbal and written communication.
- A compassionate, approachable manner when working with clients, families, and community partners.
- Attention to detail and the ability to balance multiple priorities.
- Proficiency in Microsoft Office, including Word and Excel.
- Ability to collaborate effectively with teams across departments.
- Leadership abilities, sound judgment, and a reliable work ethic focused on achieving goals.
Mandatory Requirements
- Follow all Isaiah House policies and procedures.
- Complete required in-service training and other assigned educational programs.
- Attend Family Day one Saturday per month, as scheduled by Executive Staff.
Key Responsibilities and Performance Expectations
- Coordinate sober living referrals for clients completing the program.
- Maintain accurate, current client completion and discharge dates.
- Share timely discharge updates through transition emails and designated Teams channels.
- Contact families upon client admission and maintain timely, consistent communication throughout the program.
- Prepare visit lists and verify that clients return safely from visits.
- Maintain accurate transition communications and KIPU documentation.
- Monitor incoming facility mail and ensure timely distribution to the appropriate recipients.
