Job Description
Affinity Healthcare Solutions Skilled Care is seeking compassionate, motivated, and patient-focused Speech-Language Pathologists to join our growing Home Health team across Pennsylvania.
As a Home Health Speech-Language Pathologist, you will provide individualized care to patients in their homes, helping them improve communication, cognitive function, swallowing abilities, and overall quality of life. Responsibilities
- Perform comprehensive speech-language pathology evaluations in the home setting
- Develop individualized treatment plans based on patient needs and goals
- Provide skilled speech therapy services in patients' homes
- Evaluate and treat speech, language, communication, cognitive, and swallowing disorders
- Educate patients and caregivers on treatment techniques and home exercise programs
- Collaborate with physicians, nurses, physical therapists, occupational therapists, and other healthcare professionals
- Monitor patient progress and modify treatment plans as needed
- Maintain accurate and timely clinical documentation
- Complete patient notes and required communications within agency timeframes
- Ensure compliance with agency policies and applicable regulations
- Travel to patient homes within assigned coverage areas
Qualifications
- Active Pennsylvania Speech-Language Pathologist (SLP) License
- Master's Degree in Speech-Language Pathology from an accredited program
- Certificate of Clinical Competence (CCC-SLP) preferred
- Home Health experience preferred but not required
- Strong clinical assessment and treatment skills
- Excellent communication and interpersonal skills
- Strong computer and documentation skills
- Reliable transportation and valid driver's license
- Ability to work independently and effectively manage a flexible schedule
- Commitment to delivering exceptional patient-centered care
Coverage Areas
Opportunities are available throughout Pennsylvania, including:

Montgomery County
Delaware County
Chester County
Bucks County
Lehigh County
Additional coverage areas may also be available.
