The LPN Navigator practices within the established LPN scope of practice and organizational policies and works collaboratively with the IBD RN Navigator and IBD providers. Clinical assessment, independent triage, modification of treatment plans, and other activities requiring RN- or provider-level clinical judgment are escalated to the appropriate licensed clinician.

🕒 Shift: Full-Time | Monday–Friday, 8:00 AM – 4:00 PM (no more 13 hour days, overnights, or working holidays!)Job Duties
Collaborate with the RN Navigator and Gastroenterology providers to coordinate care for patients with inflammatory bowel disease (IBD).
Conduct patient outreach and follow-up related to treatment plans, medications, laboratory and diagnostic testing, procedures, and appointments.
Monitor completion of required testing and assist patients with outstanding components of their care plans.
Reinforce education regarding medications, treatment adherence, laboratory monitoring, and follow-up instructions.
Support medication refill and prior authorization workflows for medications, biologic therapies, imaging, and other IBD-related services.
Coordinate with specialty pharmacies, infusion services, laboratories, and other services involved in IBD treatment.
Track patients receiving biologic and infusion therapies, including treatment scheduling and required laboratory monitoring.
Support pre-biologic workflows, including tracking required TB, hepatitis, and other testing.
Monitor treatment adherence and follow up on missed infusions, laboratory testing, appointments, and other components of the treatment plan.
Assist with care coordination and follow-up for IBD patients receiving TPN or other complex therapies as directed by the RN or provider.
Maintain IBD patient work queues, task lists, registries, and other program-specific tracking systems.
Document patient communication, education, navigation, and care coordination activities in the electronic health record.
Escalate clinical concerns, abnormal findings, changes in patient condition, or needs requiring nursing assessment to the RN or appropriate Gastroenterology provider.
Provide intermittent clinical support to IBD providers at a designated off-campus practice location based on operational needs.
Room patients, facilitate patient flow, obtain vital signs, perform medication reconciliation, and prepare patients for evaluation by IBD providers.
Perform other LPN-level clinical functions necessary to support Gastroenterology/IBD patient care.

QualificationsEducation:
- Current BLS certification.
- Graduate of an accredited practical nursing program.
- Current New York State license and registration as a Licensed Practical Nurse.
Experience:
- Experience in ambulatory care, chronic disease management, care coordination, patient navigation, specialty care, or a related clinical setting preferred.
- Experience in Phlebotomy required
- Ambulatory, gastroenterology, chronic disease management, specialty pharmacy, infusion, or care-coordination experience preferred.
Skills & Competencies:
- Strong organizational, communication, and patient-engagement skills.
- Ability to manage multiple patient follow-up activities and maintain accurate clinical documentation.
- Ability to work collaboratively within an interdisciplinary care team.
- Ability to communicate effectively with patients, families, healthcare professionals, and community partners.
- Ability to identify and appropriately escalate clinical concerns and barriers to care.
- Ability to travel to designated clinical and community outreach locations as required.
