This is a senior, hands-on leadership role at the heart of our Preston service. Youll oversee the coordination of care delivery across the hub, support and develop junior coordinators, and deputise for the Registered Manager when needed all while keeping the focus firmly on delivering a consistent, high-quality service for every client we support.

What were looking for
At least 12 months experience in a care coordination or supervisory role within a domiciliary or community care setting
Experience of supervising or line managing others, with the ability to motivate and develop a team
A solid understanding of CQC regulatory requirements, the Care Standards Act and the Health and Social Care Act
Strong organisational and scheduling skills, with the ability to manage competing priorities under pressure
Proficient in care management software (People Planner preferred) and Microsoft Office including Excel
Excellent written and verbal communication skills, with the confidence to liaise with clients, families, field staff and external professionals
A genuine commitment to person-centred care and delivering high-quality outcomes for service users
A full valid UK driving license with access to your own vehicle
Your day-to-day
Leading and line managing the Care Coordinator team, including supervisions and appraisals
Deputising for the Registered Manager in their absence
Overseeing scheduling and Rota management across the hub, ensuring continuity of care for every client
Mentoring junior coordinators and supporting their professional development
Leading internal audits of scheduling, care records and visit monitoring
Monitoring key performance indicators and producing reports for the Registered Manager
Acting as the first point of escalation for the coordination team
Liaising with clients, families, commissioners and healthcare professionals
Contributing to service improvement, capacity planning and onboarding of new packages
Managing on-call duties, including senior on-call cover on a rotating basis
