Position: Social Services DirectorDepartment: AdministrativeReports To: AdministratorFLSA Status: Exempt
General Purpose The Social Services Director oversees the planning, implementation, and coordination of social services for residents in a skilled nursing facility. This role ensures residents’ psychosocial needs are assessed and addressed, promotes overall well-being, facilitates care planning and discharge processes, and ensures compliance with federal, state, and facility regulations. The Director serves as a resident advocate, interdisciplinary team leader, and mentor for social services staff.

The Director serves as a resident advocate, interdisciplinary team leader, and mentor for social services staff.
ResponsibilitiesResident Care & Advocacy
Assess and document the psychosocial status and needs of each resident.Develop and implement individualized care plans that address emotional, mental, and social well-being.Provide direct support to residents and families to assist with adjustment to long-term care.Act as an advocate for residents’ rights and assist in resolving grievances or concerns.
Leadership & OversightSupervise, mentor, and support Social Services Assistants and other support staff.Serve as an integral member of the interdisciplinary care team, attending and contributing to care plan meetings.Ensure all social service activities align with person-centered care practices and facility policies.
Admissions & Discharge PlanningParticipate in the admissions process by assessing new residents and meeting with families.Coordinate discharge planning, including arrangements for home care, placement, or community services.Communicate effectively with hospitals, home health agencies, and other external providers to ensure continuity of care.
Documentation & ComplianceMaintain timely, accurate, and compliant documentation of assessments, care plans, interventions, and progress notes.Ensure compliance with federal and state regulations, including CMS, OBRA, HIPAA, and residents’ rights standards.Prepare for and participate in regulatory surveys, audits, and quality improvement initiatives.
Qualifications
Bachelor’s or Master’s degree in Social Work, Gerontology, or related field; LCSW preferred.State licensure or certification as applicable.Experience in long-term care, skilled nursing, or post-acute care settings preferred.Strong knowledge of federal, state, and facility regulations governing social services in healthcare.
Excellent interpersonal and communication skills with residents, families, and staff.Strong leadership and supervisory abilities to manage social services staff.Ability to develop and implement care plans tailored to individual resident needs.Effective problem-solving, conflict resolution, and advocacy skills.Competence in documentation, compliance, and regulatory standards.
