JOB TITLE: Physical Therapist

DEPARTMENT: McLean Rehabilitation Department

ACCOUNTABILTY: Director of Rehabilitation

WORK AREAS: McLean, Client Homes and Community

CLASSIFICATION: Non-exempt

CLINICAL COORDINATION: PTA’s, PT or PTA students, volunteers, aides

POSITION SUMMARY:

Reporting to the Physical Therapy Supervisor, the Physical Therapist provides physical therapy modalities and treatments to residents and clients of McLean as prescribed by a licensed physician. Takes all reasonable steps to ensure the quality of care of clients within the scope of this job description.

JOB FUNCTIONS:

Provides comprehensive evaluation of functional level establishes the plan of care with short and long term goals, and treatments. Comprehensive evaluation must contribute to state and federal required assessment tools.Presents this evaluation and treatment plan for physician review and authorization.

3. Implements the plan of care with special attention to the stage of development and chronological age of the client.

Identifies client and family needs for other services and refers for service, preparing for discharge needs at initial assessment.Coordinates services with the client, family and others involved in the plan of care, instructing on the treatment regime, including need for special equipment and/or services.

6. Ensures plan of care is communicated to those giving direct patient care to residents, participating in care conferences to develop the total care plan.

Evaluates client’s progress toward goals on a regular and timely basis as specified in the policies of McLean.Revises plan of care based on McLean, Medicare, Hospice, or managed care standards for those clients, as determined by requirements of the state or federal assessment tools.Prepares materials for client and family education and participates both formally and informally in teaching and education.Participates in Rehabilitation Team Meetings or Case Conferences. This may include client/family team meetings, orientation, staff development, in service/training sessions, and/or providing education to the team.Ensures documentation of treatment including date, time, modalities used, patient response, and progress towards goals. Ensure this documentation is recorded appropriately for billing with all mandatory fields completed. Provides this documentation timely and accurately.Is responsible for care and economic usage of equipment and supplies used in therapy. Provides input to the Director of Rehabilitation for equipment and supplies.Provides clinical supervision of physical therapy aids, HHAs, CNA’s who are assisting in the ADLs of our clients/patients.Participates in the performance evaluation of physical therapy assistants. Gives appropriate feedback to them and communicates issues to supervisor.Actively participates in promoting the mission and values of McLean, contributing to the increase of knowledge and/or theory, and maintaining professional development.Performs other activities within the scope of competency and license to practice as directed by the Rehabilitation Director.

STANDARDS OF PERFORMANCE:

1. Uses data gathered from client, medical record, family members, and other health team members in doing client evaluation.

2. Demonstrates competency in the application of therapy modalities.

3. Develops the plan of care based on this evaluation within the time frames specified in policy and which will meet requirements for timely signature of the physician. This includes all required comprehensive assessment tools.

4. Uses effective communication skills in interpersonal relations with clients, family members, community providers, and co-workers. Maintains confidential and professional communication for clients and staff.

5. Demonstrates commitment to the Mission and Values of McLean and Community Services Division.

6. Demonstrates knowledge of McLean’s policies, procedures, standards of care, state licensure requirements and other appropriate state/federal regulatory guidelines and performs duties according to established policies.

7. Demonstrates understanding of developmental stage and chronological age in assessing and planning for client care.

8. Uses time constructively and performs responsibilities cooperatively.

9. Evaluates patient outcomes in care and utilizes information in quality improvement activities and on-going development of standards of care.

10. Abides by the principles of Universal Precautions and infection control policies.

11. Gives appropriate feedback on employee performance for physical therapy assistants and ancillary staff.

12. Is a positive role model for professional practice of physical therapy.

13. Meets annual contact hour requirements for professional development.

WORKING CONDITIONS:

Typical Physical Demands: Requires occasional lifting of greater than 50 pounds, often combined with bending and turning. Requires ability to walk up and down stairs. Requires corrected vision and hearing within normal limits. Requires ability to travel to and from cases including the operation of a motor vehicle for home care clients and home evaluations. Requires use of office equipment such as computers, telephones, fax machines and copy machines. Requires use of medical equipment in the clinical setting. Requires eye/hand coordination and manual dexterity. Requires frequent prolonged standing, walking, and the ability to perform physical labor for an eight-hour day.

Typical Working Conditions: Work is performed in facilities, private homes, and an office environment. Contact with staff, clients, families, physicians, police, clergy, ambulance crews, pharmacists, vendors, volunteers, and students. May be required to work with uncooperative and combative residents. Exposure to communicable diseases, medicinal preparations, toxic substances and conditions common to the home environment.

EMPLOYMENT REQUIREMENTS:

Knowledge, Skills, Abilities:

Knowledge and skill in a variety of therapy modalities. Skill in communicating with clients and families. Skill in establishing positive working relationships. . Knowledge of community resources. Ability to analyze data from client outcomes to modify care. Demonstrates personal integrity and empathy and fosters such in others.

Education:

Bachelor’s degree in Physical Therapy from an accredited college or university

Certification/Licensure:

Current Connecticut State License in good standing as a Physical Therapist

Experience for Home Care ONLY:

Minimum of one year of experience. Home care preferred.
