Social Worker MSW

Are you a dedicated social worker ready to provide vital, frontline advocacy for patients across the healthcare continuum? At RWJBarnabas Health, our Case Management teams work collaboratively to support patients and families navigating the social, financial, and psychological impacts of illness. As an essential member of our interdisciplinary teams within the Psychiatry Department, you will partner directly with physicians, nurses, and allied health professionals to ensure the emotional and social aspects of patient care are championed. We view our social workers as a critical component of the care plan, redefining the patient experience through compassion, resource navigation, and teamwork. A Typical Day Might Include:

  • Providing therapy: Deliver vital emotional and administrative support to patients via individual psychotherapy and group processes.
  • Conducting assessments: Complete comprehensive clinical screenings and evaluations across assigned hospital populations.
  • Discharge planning: Formulate and execute timely, safe, and tailored discharge plans based on unique patient needs.
  • Supportive counseling: Provide emotional guidance to patients and families regarding illness, hospitalization, and transitions.
  • Resource coordination: Connect patients with critical community programs, medical equipment, and post-acute care options.
  • Interdisciplinary collaboration: Participate actively in daily team rounds to align care paths and advocate for patients.
  • Palliative care: Coordinate social work services and facilitate family meetings for goal setting and advanced care planning.
  • Continuum liaison: Serve as the primary link between internal clinical teams and external community care providers.
  • Clinical documentation: Maintain accurate, clear, and timely electronic medical charts for all patient interventions.

Job Qualifications & Requirements:

  • Education: Master’s Degree in Social Work (MSW) from an accredited School of Social Work.
  • Required Licensure: Current active Social Work Licensure in the State of New Jersey (LSW or LCSW).
  • Preferred Licensure: Licensed Clinical Social Worker (LCSW) designation is strongly preferred.
  • Experience: Previous medical social work or hospital case management experience is strongly preferred.
  • Onboarding: Successful completion of standard hospital orientation processes.
  • Coverage: Weekday days and/or evenings, plus weekend and holiday rotations alongside your team.
