As a Home Health Physical Therapist, you’ll provide individualized, one-on-one care where patients are most comfortable — at home. You’ll help patients regain mobility, improve strength, reduce pain, and achieve greater independence while enjoying the flexibility and variety that comes with a career in home health.
What You’ll DoAs a Physical Therapist, you will:
- Perform comprehensive initial evaluations and identify patient needs, functional limitations, and appropriate therapy goals
- Develop and implement individualized plans of care designed to improve mobility, strength, balance, and overall function
- Provide skilled physical therapy interventions in patients’ homes
- Evaluate patient progress and adjust treatment plans to achieve the best possible outcomes
- Educate patients and caregivers on exercises, mobility techniques, fall prevention, safety, and continued progress
- Collaborate with physicians, nurses, therapists, caregivers, and other members of the interdisciplinary care team
- Maintain timely and accurate clinical documentation in accordance with company and regulatory guidelines
- Follow infection-control and universal-precaution standards
- Help patients remain safe, independent, and successful in the place they call home
- Graduate of an accredited Physical Therapy program
- Current Colorado Physical Therapist license
- CPR certification
- Valid driver’s license and reliable transportation
- Current automobile insurance
- Home health experience preferred
- Homecare Homebase (HCHB) experience is a plus
- Strong communication, organization, and time-management skills
