Job Description
In collaboration with the patient/family, case managers, physicians and an interdisciplinary team, this position assists with complex discharge planning. The Social Work Associate is responsible to assisting patients and families to identify the barriers that impact the discharge planning process. They provide evaluation, advocacy, education and information, referral and linkage, coordination of care to assigned patients. Services are provided in the inpatient and outpatient settings. They work in conjunction with the multidisciplinary team in order to develop a discharge plan that is safe and effective for the patient.
GENERAL SUMMARY/BASIC PURPOSE OF JOB: Implementation and coordination of the transition of care needs under the direction of the Director of Case Management and Physician Advisor.
MINIMUM EDUCATION: Master's Degree in Social Work or related field.
MINIMUM WORK EXPERIENCE: Health care or related field KNOWLEDGE, SKILLS & ABILITIES:
Ability to create and analyze data, perform gap analysis. Proficient in Excel and other Microsoft Office products
Outstanding oral and written communication skills
Ability to work independently in accordance with policy and procedure.
Ability to thrive in an unstructured environment
