Description:

This position provides skill development services to children, youth, and families in home, school, and community settings to support behavioral, emotional, and social growth. The role maintains documentation, coordinates care with families and service providers, and ensures compliance with agency, state, and payer requirements. It also promotes safety, responds to crisis situations as needed, and builds positive relationships that support successful outcomes. Overall, the position helps individuals develop skills that increase functioning and independence across life domains.

Requirements:

· Provide Skill Development services to clients aged 4-21 years of age and their families in the home, school or community as outlined in the Individualized Treatment Plan (GC)

· Fulfill expectations of LPHA order. (SR)

· Develop and maintain a working relationship with the community and associated agencies. (D)

· Complete service documentation (service notes, reports and releases, etc.) within required timeframes. (SR)

· Maintain client notepad by documenting all communication or attempts to communicate with referring worker and parents or lapses in services greater than one week. (OC)

· Coordinate with other service providers involved with the individual, including, but not limited to, the individual’s mental health provider, psychiatrist, health care provider, school/educational staff, child welfare services providers, and juvenile court service providers. (OC)

· Attend and participate in all required trainings. (GC)

· Maintain caseload. (SR)

· Maintain proper norms and security of youth and provide crisis intervention as necessary. (NV)

· Attend and participate in team meetings weekly (full-time) or twice monthly (part-time). (D)

· Comply with Code of Conduct, Compliance, Confidentiality and Core Value requirements. (SR)

· Uphold agency communication standards. (OC)

· Follow Iowa Code Chapter 24.3(4.11) code of ethics, which states: staff will not engage in uninvited solicitation of potential clients; BHIS will not be provided in the home of the Intervention Specialist; BHIS will not be provided at the convenience of the Intervention Specialist, and that staff not have a conflict of interest and/or dual relationship with the individual receiving services. This includes, but is not limited to, child care provider, coach, juvenile court school liaison, family member, or personal friend of the individual. Must be at least 3 years older than the individual served who is less than 18. (SR)

· Maintain valid driver’s license and sufficient liability insurance coverage on the vehicle used for transportation of clients. (SR)

· Abide by The Health Insurance Portability and Accountability Act of 1996 (42 USC & 201 et seq.) (SR)

· All other duties as assigned. (GC)
