Traime Behavioral Health specializes in providing mental health care for persons with severe and persistent mental illness. We help persons with mental health challenges reintegrate back into the community, learn how to take medications independently, secure gainful employment, secure affordable housing, enter or re-enter into college, and regain life skills that were lost due to mental health issues. Assertive Community Treatment (ACT) is a recovery focused, high intensity, community-based service for individuals discharged from multiple or extended stays in public hospitals, or who are difficult to engage in treatment. ACT utilizes an interdisciplinary team to provide intensive, integrated, and rehabilitative crisis treatment and community support interventions/services that are available 24 hours/7 days a week. Services include but are not limited to getting help with locating supportive housing, securing employment, individual training, group training, crisis assessment, support and intervention, assistance with personal development, family counseling/training, assistance with financial management skill development, and substance abuse counseling and intervention. This position of moderate difficulty is responsible for providing a variety of case management and outreach interventions to consumers in their natural environment. Serves as advocate in assisting consumers in accessing community resources, teaching and modeling self-help and coping skills.
