Job Description
JOB SUMMARY: Provide direct support to program supervisor and consumers
JOB DUTIES:
- Review referrals and assess program eligibility
- Complete intake and initial assessment of services needed
- Assist program individuals with implementation of service plans
- Maintain required documentation
- Oversee daily operations of the Drop In Center
- Assist with Crisis Intervention
- Conduct Street Outreach as scheduled by Program Supervisor
- Follow all guidelines & procedures related to EZ Labor (clocking in & out, reviewing time
cards, requesting edit sheets, etc).
- Report any significant concerns to supervisor
- Maintain professional, courteous relationships with all consumers, staff, community
members and professional partnerships
- Attend and participate in pertinent meetings
- Maintain training requirements
- Projects and duties as assigned
JOB SPECIFICATIONS:
Must have a High School diploma and two years experience working with homeless population and/or individuals with disabilities. Time management skills a must. Must have valid drivers license and current vehicle insurance.
