JOB SUMMARY: Provide direct support to program supervisor and consumers

JOB DUTIES:

• Review referrals and assess program eligibility
• Complete intake and initial assessment of services needed
• Assist program individuals with implementation of service plans
• Maintain required documentation
• Oversee daily operations of the Drop In Center
• Assist with Crisis Intervention
• Conduct Street Outreach as scheduled by Program Supervisor
• Follow all guidelines & procedures related to EZ Labor (clocking in & out, reviewing time

cards, requesting edit sheets, etc).

• Report any significant concerns to supervisor
• Maintain professional, courteous relationships with all consumers, staff, community

members and professional partnerships

• Attend and participate in pertinent meetings
• Maintain training requirements
• Projects and duties as assigned

JOB SPECIFICATIONS:

Must have a High School diploma and two years experience working with homeless population and/or individuals with disabilities. Time management skills a must. Must have valid drivers license and current vehicle insurance.
