Position Summary:
We are currently seeking a friendly, organized, and experienced Intake Coordinator to join our growing team! This key role is the first step in welcoming new patients into our care. You will be responsible for managing all aspects of the intake process, ensuring that patients and referral sources experience prompt, professional, and compassionate service from the very beginning.

What You'll Do:

  • Receive and process new patient referrals with a sense of urgency and attention to detail

  • Communicate clearly with physicians, referral sources, patients, and families to gather necessary information

  • Verify insurance and obtain authorizations when needed

  • Coordinate care start dates with clinical and administrative teams

  • Maintain accurate, up-to-date records in the electronic medical record system

  • Collaborate with multidisciplinary teams to ensure seamless patient onboarding

  • Provide updates and follow-ups with referral sources, showing responsiveness and professionalism

What We’re Looking For:

  • Preferred: Minimum of 2 years of home health intake experience

  • Knowledge of Medicare/Medicaid guidelines, insurance verifications, and authorization processes

  • Excellent communication and customer service skills

  • Strong attention to detail and ability to multitask in a fast-paced environment

  • Proficiency in EMR systems and Microsoft Office Suite

  • Team-oriented mindset with a proactive and positive attitude
