Discharge Planner at Willis-Knighton Health System – Shreveport, Louisiana

This position is for a Discharge Planner.

The candidate chosen for this position will work closely with patients and the healthcare team with a primary focus on discharge planning, utilization of resources, evaluating the needs of the patient, timely discharge plans, ability to collaborate with and take direction from case managers, RNs, physicians, patients, patient's families, and coordinate with community referral bases to enhance optimal patient outcomes.

The candidate must be flexible, highly organized, able to complete paperwork on time, empathetic, have good communication skills and capable of multitasking in a high-stress environment.
