Job Responsibilities

Provides direct client care planning, case management, discharge planning, client care duties, and program functions under the general direction of the Crisis Stabilization-Transitional Living Center (CS-TLC) Coordinator and Clinical Supervisor for adults in the CS-TLC Program. Ensures all actions taken in carrying out responsibilities support client centered care.

Required Qualifications

Certified Medication Aide required to pass meds on sub-acute unit.

Preferred Qualifications

Possession of current valid driver's license and eligible for coverage under Mary Greeley Medical Center's insurance policy
