The Position:

We are looking for a passionate Director of Population Health, this role serves as the operational partner to the VP of Strategy & Integration, translating strategic priorities into coordinated programs across facility-based care, home and community-based services, physicians, IT, and community partners. The Director oversees development, implementation, and measurement of initiatives such as ACO participation, population health programs, clinical pathways, and cross-continuum care coordination.

The position is responsible for the following duties:

  • Leads operational implementation of system-wide population health and integration initiatives including ACO participation, clinical pathways, HCBS expansion, and care integration projects across facilities and islands.
  • Engages physicians, clinical leadership, payer organizations, and community partners to operationalize value-based contracts, clinical pathways, care coordination workflows, and population health initiatives on behalf of the VP of Strategy & Integration.
  • Coordinates development and implementation of clinical pathways (e.g., stroke, orthopedics) across care settings with clear handoff to operational teams

Requirements:

  • Bachelor's in clinical field, healthcare administration, business administration, public health or equivalent combination of education, training, and experience
  • Master’s degree in a clinical field, Healthcare Administration, Business Administration, Public Health, or related field preferred
  • Five (5) or more years of healthcare leadership, strategy, population health, or operations experience.
  • Experience in value‑based care, population health initiatives, or healthcare integration preferred.
  • Familiarity with Medicare ACO models, HCBS programs, or post-acute care settings preferred.
  • Current medical/health screening including 2 step TB clearance.
