Housing Case Manager

We are seeking a compassionate and organized Housing Case Manager to join our dedicated team. In this role, you will play a vital part in supporting individuals and families in securing and maintaining stable housing, helping them achieve greater independence and stability. Key Responsibilities:

- Assess clients’ housing needs and develop individualized housing plans

- Assist clients in locating, applying for, and securing suitable housing options

- Provide ongoing case management support to ensure clients maintain stable housing

- Coordinate with landlords, social service agencies, and community resources

- Advocate for clients to access additional services such as employment, healthcare, and financial assistance

- Monitor client progress and adjust plans as necessary to meet changing needs

- Maintain accurate and confidential case records and documentation

- Educate clients on tenant rights, responsibilities, and housing policies

Skills and Qualifications:

- Bachelor’s degree in social work, human services, or related field preferred

- Proven experience in case management, social services, or housing support

- Strong interpersonal and communication skills

- Ability to work independently and as part of a multidisciplinary team

- Knowledge of local housing resources, laws, and regulations

- Excellent organizational and time-management skills

- Compassionate, patient, and culturally sensitive approach

- Valid driver’s license and reliable transportation for transporting clients

Requirements:

In-Service Education

All new staff are required to go through orientation and complete new hire training. This training is provided by the administrator or his delegate familiar with the topic by training and experience. In addition to the orientation training for new staff, all staff participates in required 40 hours/year of ongoing trainings.

Education and/or Experience & Background Check

  • Must be a minimum of 21 years of age.

Education:

  • Bachelor’s Degree in Social Work, Psychology, Human Services, or related field preferred.

· Experience: At least one year of experience working with foster youth, probation youth or transitional age youth.

· Must pass background check, including DOJ/FBI and Child Abuse Index

· No child abuse record or allegation.

Skills

  • Strong organizational and documentation skills
  • Cultural competence and trauma informed care
  • Ability to build rapport with youth from diverse backgrounds
  • Knowledge of child welfare and community resources

Certificates, Licenses, Registrations

  • Obtain certification in CPR/First Aid.
  • Obtain certification in PRO ACT and Water Safety (if hired during summer months).
  • Must maintain California Driver License and current automobile insurance throughout course of employment.
  • Have a safe DMV driving record.
  • Submit mileage according to company policy for reimbursement.

Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision and depth perception. Have and maintain the ability to drive company vehicle.

Work Environment

Hybrid office and field-based work, includes home visits, drive to multiple sites and transportation of youth.
