About This Position
The Community Outreach Coordinator will function as part of the multidisciplinary team and will provide services to patients of all ages. Outreach is provided to ambulatory patients, as well as community members. The Community Outreach Coordinator will coordinate appointment schedules between the Population Health Team, Primary Care Providers and GVMH patients; utilize Microsoft applications to interact with Payer data; document Population Health Team activity in the electronic medical record, maintain Payer data spreadsheets, and support value-based care initiatives and population health strategies organization wide.
Education:
High School diploma or equivalent
Experience:
1–3 Years of experience preferred
