About Us:
We are a growing hospice organization in New Jersey expanding services to include a palliative care program. Our mission is to provide compassionate, patient-centered care to individuals with serious illness while supporting families and caregivers. We are seeking an experienced Nurse Practitioner to help develop and drive the plan of care for palliative care for our existing hospice program.
Position Summary:
The Palliative Care Nurse Practitioner (NP) will provide high-quality palliative care services across home and facility settings while supporting hospice operations. This role includes conducting hospice face-to-face (F2F) encounters in compliance with CMS and New Jersey regulations, ensuring accurate documentation, and supporting appropriate billing and coding practices. The NP will play a key role in building workflows and clinical standards for the palliative care program.
Key Responsibilities:
- Provide comprehensive palliative care visits, including initial consultations and follow-ups
- Manage complex symptoms (pain, dyspnea, nausea, anxiety, etc.)
- Lead goals-of-care discussions and advance care planning
- Perform hospice F2F encounters for patients entering their 3rd benefit period and beyond, ensuring compliance (F2F visits to be completed within 15 days prior to recertification)
- Clearly document clinical findings supporting continued hospice eligibility (prognosis of 6 months or less)
- Collaborate with hospice physicians to ensure timely certification/recertification
- Maintain accurate, compliant documentation aligned with CMS and NJ hospice regulations
- Demonstrate working knowledge of palliative care billing (E/M coding, time-based billing, advance care planning codes)
- Participate in development of policies, workflows, and quality initiatives for the palliative program
- Collaborate with interdisciplinary team members
New Jersey–Specific Compliance Expectations:
- Practice in accordance with New Jersey APN scope of practice and prescriptive authority regulations
- Maintain a collaborative agreement with a supervising physician, as required by NJ law
- Ensure all hospice F2F documentation meets CMS Conditions of Participation and audit standards
- Support compliance with Medicare hospice benefit requirements and NJ Department of Health expectations
- Understand documentation requirements to avoid billing denials and ADR (Additional Documentation Requests)
Qualifications:
- Active New Jersey APN license
- Board certification (FNP, AGNP, or equivalent)
- Minimum 2–3 years of hospice and/or palliative care experience
- Experience preferred in completing hospice F2F encounters but willing to train right candidate
- Strong knowledge of E/M coding, hospice documentation, and Medicare billing requirements for palliative program needs
- Excellent communication and clinical decision-making skills
- Ability to work independently in community-based settings
- Valid driver’s license and reliable transportation
Preferred Qualifications:
- ACHPN certification preferred but not required
- Preferred experience developing or expanding a palliative care program
- Familiarity with hospice EMR systems
