Shift Schedule: Monday–Friday, 9:00 AM – 5:00 PM (Flexible schedule may include weekends/holidays)Hours Per Week: 35
As a Registered Nurse for the Bronx Assertive Community Treatment (ACT) team, you will provide essential health and medical oversight for older adults with complex mental health needs. You will be responsible for conducting assessments, managing medications in collaboration with psychiatric providers, and delivering a range of rehabilitative and support services directly in the community to enhance client wellness and autonomy.

You will serve as the medical anchor of the ACT team, ensuring that clients’ physical and mental health needs are integrated and addressed.
Key Responsibilities:
Comprehensive Assessment: Conduct mental health and basic medical assessments to develop and implement individualized care plans.
Medication Management: Administer psychiatric medications, monitor for compliance, and manage potential side effects in conjunction with the Psychiatrist/NPP.
Clinical Interventions: Deliver evidence-based nursing treatments and rehabilitation services tailored to the older adult population.
Crisis Intervention: Participate in de-escalation and crisis response as needed to ensure the safety and well-being of clients.
Healthcare Coordination: Act as a liaison with primary care providers, social services, and housing agencies to ensure a seamless continuum of care.
Education & Support: Provide vital health education to clients and their families regarding mental health conditions, treatment options, and wellness strategies.
Team Collaboration: Actively participate in daily ACT team meetings, case conferences, and clinical supervision sessions.
Documentation: Maintain precise and timely progress notes, treatment plans, and medical records in accordance with legal and organizational standards.
Licensure: Current license and registration to practice as a Registered Nurse in New York State.
Experience: Prior experience working with older adults and/or individuals with mental health challenges is preferred.
Technical Skills: Computer literacy with the ability to learn and navigate program-specific electronic health record (EHR) software.
Flexibility: Willingness to work a flexible schedule, which may include weekends and holidays as required by the program.
