Case Manager – Community Based Psychiatric Rehabilitation (CBPRS)
The Case Manager provides care management and rehabilitation services to adults with substance use disorders, serious emotional disturbance, and severe and disabling mental illness under the supervision of the Program Supervisor. This role focuses on coordinating care, supporting recovery, and connecting clients to necessary services across medical, behavioral, and community systems.
A retention bonus of $2,000 is offered for this position, with $1,000 paid after 90 days of successful employment and $1,000 paid after 6 months of successful employment.
Key Responsibilities
- Assess and evaluate individuals to determine eligibility and need for care management services.
- Facilitate entry into services and monitor engagement across the continuum of care.
- Develop, coordinate, and monitor Individual Treatment Plans (ITPs) in collaboration with clients and the treatment team, ensuring plans are outcome-driven.
- Schedule and lead treatment team meetings; adjust goals and interventions based on client progress.
- Provide illness management, recovery skills training, and other recovery-focused groups as assigned.
- Coordinate access to medical, behavioral, social, educational, vocational, and recreational services.
- Ensure Medicaid screening and enrollment support when applicable.
- Maintain compliance with timelines, including treatment plan completion within 5 encounters or 21 days and annual clinical assessment updates.
- Ensure required documentation for SDMI/LOI or SUD eligibility is completed as applicable.
- Prepare reports, documentation, and billing in accordance with agency and regulatory requirements.
- Maintain required caseload and meet monthly, quarterly, and annual productivity expectations.
- Participate in supervision, training, and professional development activities.
- Travel within the service area and provide the majority of services in community-based settings; transportation of clients may be required.
Community Based Psychiatric Rehabilitation Services (CBPRS)
CBPRS services are provided separately from case management and must be billed independently. These services focus on restoring functioning and reducing disability through structured, skills-based interventions delivered in community settings.
- Provide individual and group CBPRS services focused on behavioral stabilization and skill development.
- Teach, model, and reinforce life, behavioral, and social skills aligned with treatment goals.
- Support clients in applying learned skills in real-world environments to promote independence and recovery.
- Collaborate with family members and treatment team supports to enhance outcomes.
- Adhere to service limits of up to 2 hours per day for individual and 2 hours per day for group CBPRS unless otherwise authorized.
- Maintain documentation standards, including justification for telehealth when used.
Attendance and Scheduling
This position requires flexibility to meet client needs, including variable scheduling. Changes to regular schedules will be communicated in writing. Scheduling is based on client care needs and program operations.
Requirements:Minimum Qualifications
- Bachelor’s degree in Social Work or a related human services field.
Knowledge, Skills, and Abilities
- Ability to assess client needs and respond effectively.
- Knowledge of community resources and service navigation.
- Strong interpersonal and communication skills, both written and verbal.
- Ability to build rapport with diverse populations and support recovery-oriented care.
