About This Position
Salary Range- $79.064 - $81,831
Project HOME is seeking a Street Medicine Nurse Care Manager-2 (NCM-2) to provide trauma-informed, patient-centered care to individuals experiencing homelessness and housing instability through our Mobile Medical Team and the HUB of HOPE, Monday-Friday, 8:30-4:30. This role combines direct clinical care, care coordination, street-based outreach, and mentorship within a collaborative, mission-driven healthcare team.
What You'll Do- Coordinate care with primary care, behavioral health, and specialty providers, including post-hospital and ER follow-up
- Partner with participants on assessments, goal-setting, care planning, and follow-up
- Deliver street-based and mobile health visits for individuals unable to access clinic care
- Provide education on harm reduction, recovery support, chronic disease management, and medication adherence
- Perform clinical assessments and procedures, including venipuncture, immunizations, wound care, and AIMS assessments
- Support long-acting injectable medication management and prior authorizations
- Participate in quality improvement initiatives
- BSN with an active Pennsylvania RN license and BLS certification
- At least 3 years of experience in a related environment
- Background in Public Health service, including mental health, substance abuse, and working with the unhoused
- Experience with trauma-informed care, harm reduction, and/or Medication-Assisted Treatment preferred
- Strong collaboration and relationship-building skills
