Job Description
Summary
This position is responsible for performing duties related to patient transitional care management (TCM).
In-office presence is an essential function due to the need for real-time collaboration with providers, direct interaction with patients and caregivers, and secure handling of health data in compliance with HIPAA.
Minimum Qualifications
- Minimum completion from an accredited program for medical assistants i.e., Certified Medical Assistant (CMA) or Registered Medical Assistant (RMA.), and
- Responsible for maintaining continuing education credits and renewal of certification prior to expiration.
- CPR and First Aid Certified
- Passage of a criminal background check.
- Understand and practice the ethics of confidentiality, requirements of Health Insurance Portability and Accountability Act (HIPAA).
- Valid driver’s license and reliable transportation
Knowledge, Skills, and Abilities
- Computer skills using Microsoft (MS) Excel and SharePoint, MS Word, MS Outlook, MS Teams, Acumen, and various data management platforms used by partners of Eastern Nephrology Associates.
- Ability to function well while involved in multiple task assignments.
- Ability to understand and follow oral and writer instructions.
- Ability to meet set deadlines.
- Possess a willingness to accept orders and perform repetitive tasks.
- Skill in organizing resources and establishing priorities.
- Excellent verbal and written communication skills required.

Essential Functions
Essential Functions: All essential functions listed below are expected to be performed on-site. ENA has determined that these duties cannot be effectively or securely executed in a remote environment without compromising patient care, data privacy, or operational standards.
- Collect, track, and record patient information relating to dialysis starts 10%
- Track, collect and communicate data for patients at risk for end stage renal disease. 10%
- Collect, track, and communicate information relating to kidney disease education 10%
- Other duties as assigned 10%
- Continuous monitoring of various informational sources (i.e., Health Information Exchange (HIE), hospital systems, etc.) to identify patient activity relating to discharge from an inpatient setting. 15%
- Provide interactive contact within two business days of discharge from an inpatient setting to perform required components of the TCM outreach. 15%
- Provide coordination of patient appointments for TCM face to face services, hospital follow-up, etc. 10%
- Continuous monitoring of various informational sources (i.e., HIE) to identify patient activity relating to recurrent emergency room use and repeat hospitalizations. 10%
- Provide patient care services in the clinic as assigned. 10%
Physical Demands
- Hearing, color vision, depth perception, field of vision, balancing, handling, and talking.
- Extended periods in a stationary position.
- Repetitive motion such as entering data into computer-based programs.
- Position self with full range of body motion, manual dexterity, and eye-hand coordination.
- Lift or move up to 10 pounds. Employee is expected to use appropriate ergonomics and tools such as hand carts for heavier loads.
Work Environment
Work is normally performed in a typical interior office environment. The noise level is usually moderate. Some office environments have stairs.
The work schedule is Monday-Friday, 8:00 am to 5:00 pm with a one-hour lunch. Some after-hours work may be necessary to achieve the goals of the organization. Travel to other sites of Eastern Nephrology Associates may be required.
