Job Description
Homebridge serves a high risk, high needs, complex-diagnosed population with a combination of domestic, personal, and paramedical homecare functions funded through IHSS. Our work allows an often-underserved population to live safely in their community. Homebridge offers a dynamic, fast-paced work environment with 350+ unionized field workers and 90+ internal office staff.
The Care Transitions Coordinator provides quality home care services to seniors and people with disabilities, thereby improving their quality of life and enabling them to remain living at home independently and safely.
Personal Care Services
- Ambulation – includes assistance with walking or moving the client from place to place inside the home, changing locations in a room, moving from room to room to gain access for the purpose of engaging in other activities. It does not include movement solely for the purpose of exercise. May include pushing the client in a wheelchair and/or operating a Hoyer Lift
- Bathing – includes cleaning the body using a tub, shower, sponge bath, or bed bath, including getting a basin of water, managing faucets, getting in and out of the tub or shower, reaching head and body parts for soaping, rinsing, and drying
- Bowel and bladder care – includes emptying of catheter or colostomy bags, assistance with bed pans, application of diapers, changing rubber sheets, and assistance with getting on and off the toilet. Also includes emptying the commode, managing the clothing and wiping and cleaning the body after toileting, application of diapers and/or disposable barrier pads
- Dressing client – including putting on and taking off clothing, fastening and unfastening garments and undergarments, and special devices such as back or leg braces, corsets, and elastic stockings/garments
- Range of motion exercises – includes general supervision of exercises, which have been taught to the client by a licensed therapist or other health care professional
- Feeding and assurance of adequate fluid intake – this includes related assistance to clients who cannot feed themselves or who require assistance with special devices in order to feed themselves or drink adequate liquids. This includes reaching for, picking up, grasping utensils, cup to mouth, manipulating food on a plate and cleaning face and hands as necessary following meals
- Grooming – includes hair combing and brushing, shampooing, oral hygiene, shaving and fingernail and toe care (excluding cutting with scissors or clipping toenails
- Assistance with self-administration of medications; includes reminding the client to take prescribed medications when they are to be taken
- Menstrual care limited to application of sanitary napkins and external cleaning
- Prosthesis care and assistance
- Assistance with self-administered oxygen and cleaning of IPPB machines
- Rubbing of skin with lotion
- Turning client in bed using Hoyer Lift where necessary
- Paramedical services as assigned and trained
Domestic Services
- Sweeping, vacuuming, and washing floors
- Cleaning kitchen, including: counters, sinks, oven and stove, as well as defrosting and cleaning the refrigerator
- Cleaning bathroom
- Storing food and supplies
- Taking out garbage
- Dusting and picking-up
- Changing bed linen and making bed
- Menu planning and preparing meals for immediate consumption as well as for storage to be consumed later
- Meal clean-up, including: washing and drying dishes, pots, utensils, and culinary appliances and putting them away
- Laundry services including: carrying laundry to laundry facilities, routine washing and drying, mending, ironing, folding, and storing clothes on shelves or in drawers. Also, filling soap containers, reaching into machines, handling wet laundry, operating machine controls, and hanging laundry to dry if dryer is not routinely used.
- Reasonable shopping and errands
Other Services
- Accompanying clients to appointments with doctors, dentists and other health care practitioners and for fittings for health-related appliances and special clothing, where Medi-Cal will not provide transportation. May use public transportation and/or taxicabs. May include pushing client in a wheelchair.
- Heavy cleaning (when authorized by a supervisor) involves working collaboratively with a colleague to perform deep cleaning tasks within the designated scope. This may include removing hazardous debris and dirt, thoroughly cleaning cupboards (inside and out), cleaning beneath and behind furniture, and other approved domestic chores.
Other Responsibilities
- Reporting – Informing your supervisor as to changes in the client’s health or safety.
- Recommending ways to improve a client’s safety and comfort to your supervisor.
- Completion of Homebridge’s Home Care Professional training course
- At least 6 months of previous home care experience required
- One year highly preferred
- Experience working with clients who have been previously hospitalized for long periods of time highly preferred
- Combination of related work experience, education, and/or training
- Basic arithmetic
- Knowledge of basic domestic chores including cooking and cleaning
- Able to perform personal care services
- A sincere desire to help people in need and provide them compassionate care
- Have a patient and caring personality
- Must be dependable and able to follow directions
- Able to work and travel in all areas of San Francisco
- Ability to effectively communicate and understand directions
- Bilingual skills in Spanish, Russian, Cantonese, Japanese, or Tagalog are preferred
- Successfully complete the Homebridge Home Care Provider training required
- Experience working with target populations, including people of diverse cultural, education, socio-economic, and linguistic backgrounds, and knowledge of related issues preferred
This position is primarily in the field and includes frequent travel - must be able to travel on foot or via public transportation to all parts of San Francisco County for performance of job duties. May occasionally be exposed to outside weather conditions while traveling to clients. Some exposure to physical risk. Exposure to client apartments, rooms, homes and other venues so that services can be provided, client homes may have pets/pet dander and/or cigarette smoke. May include contact with clients with mental health and/or personal hygiene issues. Schedule would be typically based in the evening or overnight (as needed) with at least one weekend day availability. A typical 40 hour work week with possibility of overtime as needed.
Care Transitions Coordinator is responsible for domestic services and personal care, therefore it is essential that all Care Transitions Coordinator is able to meet the following physical demands:
- Able to be physically active for duration of work shift, excluding breaks
- Able to lift up to 25 lbs.
- Able to carry 25 lbs. up one flight of stairs
- Able to walk up and down a minimum of two flights of stairs
- Able to bend down and forward at the leg and spine
- Able to bend downward and forward at the waist
- Able to kneel on one or two legs
- Able to stand for extended periods of time
- Able to extend hand(s) and arm(s) in any direction
