Job Description

BASIC FUNCTION:

Evaluates and documents functional status of patients based upon strengths, needs, abilities and preferences and establishes and documents treatment based on evaluations in accordance with accepted Cardiopulmonary Rehab programs.

JOB DUTIES:

Clinical
1. Performs assessments, evaluations and re-evaluations; establishes treatment programs for all assigned Cardiac Rehab & Pulmonary Clients.
2. Participates in development and implementation of patient exercise prescriptions.
3. Completes all required documentation for evaluations, progress notes, and discharge in Q-tel system and/or paper format.
4. Updates the referring practitioner on the patient’s progress, through progress reports.
5. Assists with the organization’s performance improvement plan.
6. Assists with housekeeping duties in department.
7. Assists with inventory and stocking of department supplies.
8. Follows all policies and procedures in appropriate manuals, employee handbooks, and safety manuals.
9. Communicates effectively with all members of the interdisciplinary team.
10. Evaluate and recommend new equipment, techniques, and procedures as required. May be involved in development and implementation of new policies and procedures.
11. Recognizes and practices the highest level of integrity and honesty in maintaining patient confidentiality.
12. Demonstrates a working knowledge of Cardiac & Pulmonary disease processes.
13. Updates educational materials, policy and procedures, and education materials as needed
14. Performs all duties assigned by Director, Clinical Coordinator, or Medical Director.
Educational
1. Assists in educational instruction of patient according to Cardiac Rehab & Pulmonary Program.
2. Presents in-services for staff education.
3. Attends special courses, conferences and seminars outside the hospital for continuing education as approved by Manager and Administration.
4. Performs duties as a Clinical Instructor with students assigned.
5. Participates in staff proficiency testing.
6. Responsible for checking e-mails and on-line learning assignments on a regular basis.

AGE APPROPRIATE CARE:

Colleague has been instructed in providing customer service and on interacting with age groups as related to the department's general client population: including use of appropriate interventions, communication, and skills to match the age and/or abilities of the patient/family.

EDUCATION, TRAINING & EXPERIENCE:

Bachelor of Science Degree in Exercise Physiology/Science or Master of Science in Exercise Physiology/Cardiopulmonary Rehab. BLS through American Heart Association and EKG interpretation required within 6 months, ACLS through American Heart Association preferred. Must be computer literate to complete task(s) as required. Such as MediTech (Hospital Support Software), Halogen (performance appraisal system) Relias (education system), ability to retrieve e-mails, complete online enrollment for health insurance/benefits and any other programs as assigned.

PHYSICAL REQUIREMENTS:

• Able to stand/walk for up to full scheduled shift per day.
• Able to bend to reach items on floor or bedside level.
• Able to reach overhead.
• Able to reach over a bed far enough to give care and turn patients.
• Able to push or pull patients or necessary equipment (i.e. wheelchair, EKG machine, code cart).
• Able to lift 50 lbs. intermittently, unassisted.
• Able to move/transfer and ambulate patients unassisted and/or assisted as necessary using good body mechanics or appropriate techniques (i.e. slide board, total lift).
• Able to lift, hold, and/or support a patient’s extremity or trunk for a period of time during a procedure.
• Able to climb onto step stool and to apply a minimum of 25-pound pressure/force for compressions during CPR.
• Able to wear personal protective equipment for an extended period of time, as necessary, (i.e. mask, goggles, etc.).
• Able to turn and twist as necessary.
• Able to squat, kneel, or crawl as necessary.
• Ability to see clearly and able to distinguish appropriate rhythms from a minimum distance of 6 feet.
• Able to hear normal speaking voices.
• Able to communicate verbally and over the telephone in a clear speech.
• Able to write clearly.
