Senior Product Manager (AI Care Management)

This role sits at the intersection of healthcare delivery, artificial intelligence, and product innovation, focused on building intelligent systems that support frontline care teams working with complex patient populations. You will define and shape AI-powered agent experiences that reduce administrative burden and improve care coordination across medical and social services. Operating in a highly regulated, mission-driven environment, you will translate real-world clinical workflows into scalable product capabilities. The role requires close collaboration with clinical leaders, engineers, and operations teams to ensure that technology meaningfully improves outcomes for vulnerable populations. You will own the roadmap for a next-generation AI agent platform designed to augment human care managers rather than replace them. This is a high-autonomy role where product decisions directly influence patient outcomes, system efficiency, and care quality at scale.

Accountabilities:
  • Define and drive the product vision and roadmap for AI agent-based care management tools from discovery through launch and iteration.
  • Partner closely with clinical leadership and care teams to deeply understand workflows, pain points, and opportunities for AI-driven support.
  • Translate complex clinical and operational needs into clear product requirements, decision documents, and execution-ready specifications.
  • Design evaluation frameworks, success metrics, and feedback loops to measure AI performance and real-world clinical impact.
  • Collaborate with engineering and data science teams to build, test, and iterate on AI/ML-powered features in production environments.
  • Ensure alignment across clinical operations, engineering, and product stakeholders on priorities, outcomes, and roadmap direction.
  • Drive continuous improvement of AI agent systems by analyzing performance, user feedback, and operational outcomes.
Requirements:
  • 4+ years of product management experience, including at least 2+ years in healthcare, care delivery, or regulated environments.
  • Proven experience owning end-to-end product areas and delivering measurable outcomes across cross-functional teams.
  • Hands-on experience building or shipping AI/ML-powered products, including defining evaluation criteria and success metrics.
  • Strong understanding of clinical workflows, care management systems, or health plan operations.
  • Excellent written and verbal communication skills with the ability to simplify complex technical and clinical concepts.
  • Experience working within regulatory constraints such as HIPAA or value-based care frameworks.
  • Strong analytical thinking, structured problem-solving skills, and comfort operating in ambiguous environments.
  • Bonus: Experience with Medicare Advantage, Medicaid programs, ECM, D-SNPs, or agentic AI systems in healthcare.
