Care Manager - Outpatient EMG in Schenectady, New York at Ellis Medicine

The Care Manager (CM) oversees and provides access to all of the services an individual needs across the care continuum to assure that they receive everything necessary to stay healthy, out of the emergency room and out of the hospital. The Care Manager establishes a professional trusting relationship with clients and community providers, and ensures coordination of services and positive outcomes. She/he collects critical information through ongoing assessments for the establishment and monitoring of a person centered care plan that integrates a comprehensive array of clinical and non-clinical services, in coordination with an interdisciplinary team.

EDUCATION AND EXPERIENCE REQUIREMENTS:

• Registered Nurse with active NYS license; Bachelors’ preferred
• Ability to work well with individuals of various backgrounds, age, ethnicities, life positions, and socio-economic statuses
• Strong clinical and assessment skills; outstanding communication skills
• Computer literacy
• Highly organized and able to manage multiple priorities appropriately
• Independent problem solving skills
• Able to work collaboratively and build enduring relationships with providers, clients and the multidisciplinary team
• Requires the ability to drive, must possess a valid driver’s license and automobile that is insured in accordance with New York State Requirements
• At least 5 year experience as a care manager with focus on community

RESPONSIBILITIES OF THE POSITION:

• Meet directly with clients, their families and providers in office or home settings to review clients’ functional status, medical and social resource needs.
• The Care Manager will assist patient and family with support and education about availability of identified services, and assess decision making, coping skills, and barriers to managing their healthcare needs
• Analyze data and use it to improve care delivery
• Organize interdisciplinary care teams and act as facilitator to ensure that clients’ needs are met.
• Prepare individual care plans on behalf of clients and for use by Primary Care Physician (PCP), specialty care provider and/or other community providers.
• Coordinate and arrange for all services required by a client in accordance with the individual care plan.
• Monitor client’s health status and ensure that client is receiving all necessary medical and supportive services.
• Collaborate with other Care Managers to reduce variations in clinical practice, identify opportunities for systems’ improvement, and develop policies and procedures relevant to practice
• Must be able to work closely with others and work with patients in both routine and stressful situations related to the patient’s medical condition. The Care Manager adhere to policies and procedures related to:
• Mandatory education and Annual health assessment
• Ensure that continuing education via Health Stream is completed annually and that he/she will complete annual health assessment form as required, and maintains current inoculations.
• Will prepare and maintain a variety of records and reports of client progress and programs.
• Must be able to incorporate/utilize the translation resources available to ensure that language does not become a barrier of care.

PHYSICAL REQUIREMENTS

The position is located indoors under normal working conditions. Should be able to push/pull 25 lb., lift/move 15 lb. from floor to table, be able to perform moderately difficult manual manipulations such as using a key board, writing and filing for extended periods of time, must be able to perform tasks which require hand-eye coordination such as data entry, typing and using photo copiers. Mobility requirements may include the ability to sit at a computer terminal or work station for a prolonged period of time in addition to being able to squat, stand and walk for a reasonable length of time and distance. Sensory requirements include the ability to articulate and comprehend the spoken English language in addition to being able to read the English language.

Salary Range: $29.44-$44.16 /hour Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
