Description:

Purpose:

Provide assistance and constructive guidance to individuals in completing daily tasks and activities. Provide training in life/social skills. May work at a specific organizational facility, an authorized community/program location and/or client home. Works under general supervision. This job description is not an exhaustive listing; other duties may be assigned or implied.

  1. Ensure that each client receives the highest level and quality of supportive care and guidance.
  2. Ensure that all clients are treated with dignity and respect, and experience effective assistance in completing daily tasks/activities.
  3. Provide services in timely and focused manner to meet/exceed the organization’s goals and expectations.
  4. Follow treatment plans and supervisory directives to support the achievement of client-specific goals.

Operational Excellence

  1. Ensure that all assistance complements program intent and outcomes. Comply with all relevant laws, regulations, standards, and best practices.
  2. Ensure any Personal Health Information concerning a client is not disclosed, other than as permitted by HIPAA and/or applicable federal regulations.
  3. Actively engage supervisor in problem-resolution and guidance in performing work duties to ensure client’s best welfare.
  4. Ensure regular and consistent communication/feedback with supervisor(s) regarding scheduling, completing documentation, practical application of training, problem-solving and resolution and seeking guidance in performing duties.

Relationships

  1. Develop and maintain respectful, courteous, and professional relationships with all clients and their families (as appropriate). Work closely with all others involved in delivery of services to ensure that client needs are being met.
  2. Maintain positive, effective working relationships with other staff members and external community partners.

Leadership and Workplace Culture

  1. Uphold and ensure all activities and conduct are compliant with internal policies, as well as all relevant laws and regulations.
  2. Model the values and the mission of the organization at all times.
Essential Functions

1. Provide transportation and accompany clients to and from activities and/or appointments to provide medically necessary direct care and support, as directed.

2. Assist in providing structured and constructive activities that support the client’s treatment.

3. Assist clients with learning and practicing daily living and social skills.

4. Assist clients with problem-solving daily living situations.

5. Complete all required documentation thoroughly, on time and according to agency standards.

6. Provide above services on an individual basis or in a group setting as required.

7. Engage individuals in a positive, constructive, respectful, and hopeful manner.

8. Recognize and demonstrate sensitivity to cultural and ethnic differences.

9. Help others succeed by demonstrating exceptional interpersonal skills and the ability to work with persons with severe and persistent mental illness and/or serious emotional disturbance, and the ability to react effectively in a wide variety of human service situations. Effectively manage conflict.

10. Interact positively with other staff, members of the agency and team, including active participation in supervision activities, team meetings and training, etc.

Requirements:

Qualifications:

Experience: Prior experience providing effective, efficient healthcare/welfare services work preferred.

Education: bachelor’s degree in a social services field preferred but will consider high school diploma. G.E. D. or associate degree with experience in relevant area.

Skills & Abilities:

  • Emotional capacity to control and manage self if clients act out or behave in a verbal or physical manner.
  • Emotional and physical capacity to manage clients who may act out or behave in a verbal or physical manner.
  • Ability to effectively use all devices, equipment, computer systems required to fulfill the essential functions of this position.
  • Ability to establish and maintain good relationships with individuals.
  • Demonstrated good interpersonal and communication skills.
  • Communicate directly and effectively in a cooperative manner.
  • Ability to relate and work in an innovative manner with clients at risk of hospitalization.
  • Relates a positive approach to complex mental health issues.
  • Flexibility in assigned working hours, including evening and/or weekends.
  • Ability to be sensitive to cultural and ethnic differences.
  • Familiarity with mental health theory and practice

Physical/Environmental/Other:

  • Is 18 years of age or older.
  • Valid driver’s license
  • Must have reliable transportation.
  • Must meet all pre-employment screening requirements and required organizational training.
  • Pass a background and security check.
