Patient Engagement Coordinator (LPN) in Fort Myers, Florida at Preferred Care Home Health

Preferred Care Home Health is seeking a motivated LPN to join our Administrative Services team as a Patient Engagement & Recapture Coordinator.

In this role, you’ll use your clinical knowledge to identify current and former home health patients following hospital or ER visits, reconnect them with services, and help prevent gaps in care. You’ll work closely with our Intake, clinical, and Account Executive teams to ensure patients receive a timely and seamless transition back to home health.

• Monitor hospital systems and CarePort for patient hospital/ER encounters.
• Contact patients and caregivers to support re-engagement with home health.
• Review clinical documentation, orders, diagnoses, medications, and discharge information.
• Coordinate required documentation and timely handoffs to Intake and Account Executives.
• Document outreach and outcomes while maintaining HIPAA compliance.
• Identify and escalate clinical or safety concerns appropriately.

• Current, Florida LPN/LVN license.
• Healthcare experience required; home health or post-acute experience preferred.
• Strong clinical knowledge and communication skills.
• Organized, proactive, and comfortable working independently.
• Experience with EMRs, CarePort, WellSky/Kinnser
